You could walk in for your Covid vaccine booster on Monday Sept 29th and Tuesday Sept 30th between 10am and 2 pm . You could also book an appointment online at www.firstavepharmacy.com

There is one week left to submit an application form for the Government of Ontario's Long-Term Care Home Rate Reduction Program in order to qualify for the *full year term.

If you miss the September 30th deadline for the full year term which runs from *July 1 - June 30th of the following year, applications can still be submitted at any time. The rate reduction will begin to apply for up to 90 days before the date the form was submitted.

To make sure you receive a rate reduction for as many days as possible, submit your application within your first 90 days at your long-term care home and/or remember to re-apply annually within the July 1-September 30th period.

NOTE: Only the resident, the resident's POA of Property, or the resident's Guardian of Property may apply. Substitute Decision Makers (SDM) of the resident are not legally eligible to apply on their loved one's behalf.
