🌟 We’re Hiring! 🌟

The North Lambton Community Health Centre, in partnership with the Canadian Mental Health Association, is currently seeking applicants for the following position:

0.8 FTE Nurse Practitioner
HART Hub (30 hours/week)

The Nurse Practitioner (NP) will play a key role in providing comprehensive, client-centred care to individuals experiencing homelessness and those facing concurrent mental health and addiction challenges. As part of a multidisciplinary team, the NP will deliver primary and episodic care, offer mental health and addiction support, and help connect clients to housing and essential social services.

This position supports the Ontario government’s Homelessness and Addiction Recovery Treatment (HART) Hub initiative, which focuses on improving access to care and strengthening service coordination for vulnerable populations.

North Lambton Community Health Centre is seeking a Registered Social Worker (RSW) to join our team on a 17-month contract (22.5 hours per week), commencing May 1, 2026.

The Registered Social Worker will provide counselling and other therapeutic services to support the psychosocial needs of clients accessing services at the Community Health Centre.
