Primary Care Nurse Practitioner Full Time Permanent
Annual Salary $125.477
Superior Family Health Team is seeking a highly motivated, self-directed individual with strong interpersonal skills to join our multi-disciplinary team-based clinic.
Working within the context of a multidisciplinary team, the Nurse Practitioner (NP) will work collaboratively with an allied health care team of professionals within the Family Health Team and with our community partners. As a vital member of the patient focused team, the NP will provide primary care services in conjunction with all health care professionals.
The Primary Care NP will provide comprehensive primary care to a roster of patients, also providing acute/episodic care, health promotion, disease prevention and chronic disease management in a primary care setting.
Assist in the ongoing development of primary care programs, resources in collaboration with other members of the FHT and other partners.
Participates and collaborates with other health care providers in
the provision of patient care.
Participates in providing education to nursing staff to enhance
nursing practice.
Collect and maintain accurate data and information in
accordance with the Personal Health Information Act (PHIPA) and
the requirements of any regulatory body standards.
Effective listening, observation, and facilitation skills to effectively assess the patient and provide appropriate treatment and consultation.
Other duties as assigned.
Qualifications
Current registration with the College of Nurses in Ontario in the Extended Class in good standing
The ability to work in a rapidly changing environment.
Demonstrated ability to organize, prioritize and multitask
efficiently.
A current Criminal and Vulnerable Sector Reference Check.
Valid First Aid & CPR Certificate

Superior Family Health Team was excited to welcome Dr. Linda Lee and her amazing team for Nurse Practitioner Boot Camp MINT training session.

Our SFHT Team, Dr. Lee's Team and our community partner Alzheimer Society Sault Ste. Marie & Algoma District (pictured) had an amazing 2 days of in depth sessions.

Dr. Linda Lee is a Care of the Elderly family physician, Director of the Centre for Family Medicine Memory Clinic and Executive Director of MINT Memory Clinic, Schlegel Research Chair in Primary Care for Elders, and Associate Clinical Professor in the Department of Family Medicine at McMaster University. She has developed the Multi-specialty Interprofessional Team (MINT) Memory Clinic model and Training Program which has assisted over 120 primary care settings in Ontario, BC, Alberta, Saskatchewan, and New Brunswick to develop new MINT Memory Clinics.. Dr. Lee’s research interests focus on improving health care for older adults living with dementia and other complex chronic conditions associated with aging.

Superior Family Health Team's Memory Clinic has been established since 2011 and is open to community members.
