The Ontario Government announced yesterday that parents can now start vaccinating their children who are between the ages of 5 - 11 years old. Children must be turning five years old by the end of 2021, and born in 2016.

Please note, the Shelburne Centre for Health does NOT have any vaccination clinics scheduled at this time and you will be required to book an appointment with Public Health or a select pharmacy.

Due to the overwhelming response from our community, Shelburne Centre for Health will be hosting a rostering clinic for patients that require a new Physician.
Priority will be given to patient's who do not currently have access to a physician. If you have added your contact information to our waitlist, we would encourage you to visit us to enrol with your new physician.

Looking for a Medical Exemption for the Covid-19 Vaccination?

Currently there are only two valid reasons for a medical exemption from the COVID-19 Vaccination.

• allergic reaction to a component of the vaccine within an individual, which must be confirmed by an allergist or immunologist.

• Individuals with physician-diagnosed myocarditis or pericarditis following their first COVID 19 vaccination.

If you wish to decline for non-medical reasons, there are currently no exemption affidavits for adults and as such your primary care physician cannot provide documentation.

Please contact the clinic directly at 519 925 0017 if you fall into either of these categories and require an exemption.

Ontario to Require Proof of Vaccination in Select Settings

As of September 22, 2021, Ontarians will need to be fully vaccinated (two doses plus 14 days) and provide their proof of vaccination along with photo ID to access certain public settings and facilities.
For a list of settings and facilities that will require proof of vaccination, please see the link below.
There is an additional link on this page, that will allow you to reprint your vaccination receipt should you need it.

As of September 22, proof of full vaccination will be required to attend some settings. You can download save or print your vaccine receipt from covid19.ontariohealth.ca. **MODERNA VACCINE CLINIC: THURSDAY, JULY 29TH**

Shelburne Centre for Health has limited spots available for tomorrow, Thursday July 29th.

Shelburne Centre for Health will be holding a vaccine clinic tomorrow, Tuesday April 20th.
Following the direction of our local Public Health Office and the Ministry of Health, we are vaccinating patients 55 and over at this time.
For patients of the Dufferin Area Family Health Team (excluding Shelburne Centre for Health) we would ask that you contact your Physician's office to book an appointment.
If you are a patient of Shelburne Centre for Health and would like to receive your Covid-19 Vaccine, please forward the following information to vaccine@scfh.ca and include the following information
- Full Legal Name
-Year of Birth
**This clinic is for 1st dose vaccines only**
Please be aware that our call volumes are substantial at this time, therefore we are requesting that patients do not call the centre for a Covid-19 Vaccination appointment unless you do not have access to email.

In preparation for the possibility of Covid-19 vaccine clinic's at the Shelburne Centre for Health, we are requesting those patient's of the clinic with birth years between 1957-1961 to forward their email address to us at Vaccine@scfh.ca and include the following information:
-Your Full Legal Name
-Year of Birth
If you have family members that fall into this age group who do not have access to Social Media but are interested in receiving the vaccine, please feel free to assist in forwarding their information to us via the email address provided above.
Information regarding dates of clinics, and how to register for these clinics will be provided to you via the email address you supply as it becomes available.
Contacting the clinic with this information via email rather than by telephone will expedite this process and ensure that we have the necessary contact information available for all upcoming vaccine clinics, however as quantities of the vaccine being released at this time are unknown.
Appointments will be provided on a first come first served basis.

With the announcement from Premier Doug Ford this afternoon, regarding Primary Care settings administering the Covid-19 Vaccine, Shelburne Centre for Health would like to advise that we do not currently have vaccines available on site.
We will be notified by Public Health when and if we will be administering these vaccinations, and in turn will notify our eligible patients.
Our direction currently is for those eligible patients to register on-line with your local Wellington Dufferin Guelph Public Health Agency wdgpublichealth.ca/register OR call the WDG Public Health Registration and Booking Help Line at 1-800-265-7293 ext 4506. Please note that call volumes are extremely high at this time.
