The Perth and Smiths Falls District Hospital is a fully accredited health care facility located on two sites serving a catchment population of approximately 44,000 residents within the tri-county region of Lanark, Leeds and Grenville. The Perth and Smiths Falls District Hospital was incorporated in 1995 following the amalgamation of the Smiths Falls Community Hospital and the Great War Memorial Hospital of Perth District. The hospital is located in the South East Local Health Integration Network (LHIN #10). The Perth and Smiths Falls District Hospital has a long history in the provision of high quality health services to its community. The Hospital’s mission is to provide quality care based on the principles of total quality management for acute and chronic in-patients, as well as provision of emergency and ambulatory care. The Hospital is also committed to the development and support of health promotion programs in cooperation with other professional, community, and planning agencies. Vision

New Message, Same Number – Phone System Update Coming Soon!

The Perth and Smiths Falls District Hospital is upgrading our phone system to serve you better!

Beginning Tuesday, November 4, 2025, when you call the hospital, you will hear a new message designed to help direct your call more efficiently. Please note that many departments now have new extension numbers, so be sure to listen carefully to the updated message and menu options.

Our main hospital phone numbers remain unchanged, so you can continue to reach us as usual. If you are unsure which department to contact, just dial “0” for Switchboard, and our team will assist you.

***If your situation is an emergency or you require immediate medical care, please call 911.***

This update is part of our ongoing commitment to improving communication and accessibility for our patients, families, and community.

There was lots of spooky spirit today at the Perth & Smiths Falls District Hospital - Corporate.

Thank you to everyone who brought their spooky spirit today! From creative costumes to impressive pumpkin carvings, you made this Halloween celebration a great success.

Special thanks to the Workplace Wellness Committee and Dayna in Physiotherapy for sponsoring this year’s candy carts and to the Senior Team for delivering the goods!

Shout out to our Physiotherapy departments and Finance for their amazing pumpkin carving skills. Your festive creations were a hit!
