The Perth and Smiths Falls District Hospital is a fully accredited acute care health care organization located on two state-of-the-art sites. The hospital is situated in Algonquin Territory in the heart of the Rideau Canal region and proudly provides health services to more than 75,000 people in the surrounding communities. The hospital delivers a broad range of acute and secondary services and programs such as emergency care, diagnostic imaging (CT scan and MRI services) obstetrics, general and specialty surgical services, dialysis as well as laboratory and infection control services. The Perth and Smiths Falls District Hospital was incorporated in 1995 following the amalgamation of the Smiths Falls Community Hospital and the Great War Memorial Hospital of Perth District. The hospital is located in the South East Local Health Integration Network (LHIN #10). The Perth and Smiths Falls District Hospital has a long history in the provision of high quality health services to its community. Mission:

March is Nutrition Month – “Nourish to Flourish”.

The Perth and Smiths Falls District Hospital celebrates the amazing team of dietitians and services they make available and provide to patients. Dietitians are important members of the patient’s health care team.

Dietitians may see you to help with selecting a specialized diet and counselling you about the types of food to eat. If you have diabetes, or you are at risk of developing diabetes, talk to a member of your health care team to be referred to a diabetes education program. A doctor’s referral is necessary to access the hospital dietitian services.

Enhancing Out Patient Cardiology Care at PSFDH

We are making important changes to ensure we continue providing the highest quality care to our patients.

Out Patient Cardiology Services will be consolidated at the Perth (GWM) Site to:
• Improve the overall patient experience by having services and providers in one location
• Support safety and continuity of care
• Reduce duplication of equipment and resources
• Use clinic and clinical space more effectively

Echo services are not changing and will be provided at both sites of the Perth and Smiths Falls District Hospital.

If you already have a cardiology appointment, it will now take place at the Perth (GWM) Site. If you need to book a new appointment, you will be directed to schedule it at the Perth site.
