⚠️ Attention: Community Pharmacies!

💊 ISMP Canada recently received a report of an event involving a mix-up between the medications trimipramine and trimebutine.

These two medications are used for very different indications, and a mix-up can potentially lead to patient harm.

To prevent similar incidents:

✅ Perform a clinical check to ensure the medication, indication, and dose are appropriate for the patient

✅ Engage the patient in a discussion about their medication as a final check at pick-up to ensure that the medication, dose, and directions match the intended indication.

Learn more about look-alike/sound-alike (LASA) mix-ups here: https://ismpcanada.ca/bulletin/mix-ups-between-diclectin-and-dicetel/
