This brilliant, kind, funny, talented, beautiful & incredible woman turns 50 today on Valentine's Day. She wrote me a poem for my 50th but I'm not much of a poet. All I can write is how truly blessed I am - and all of my patients, her family & friends - to call her my friend.

To All Patients of Dr. Roxanne Cooper
To the 1200 wonderful people whom I have had the privilege of caring for in the past 17 years:.

Please be advised that after much consideration I am closing my general family practice in 2025.

Many people are aware that I have been reducing my working hours due to personal & medical issues. Despite many changes, I am no longer capable of providing timely and comprehensive care to everyone in my practice. I sincerely regret the impact this will have but believe that this change is important for my own personal health & well-being.

1. I have been unable to find a replacement family physician for my patients. Recommendations for finding a new provider are below and other resources are on the second page.
2. Effective April 1, 2025, I intend to stop practicing general family medicine, and instead open a referral practice focused on ADHD management and pap clinics.
3. We have no appointments available until 2025 and are now unable to see patients for new concerns.
4. You may book a final in-clinic appointment before April 1st to discuss your transition to a new provider.
5. Medication refills will be extended until 2026 if appropriate. Please allow at least one week's notice.
6. I will follow up on the results of all investigations and contact you when they become available.
7. We will provide copies of medical charts (or chart summaries) upon request for a fee.
8. All patients should register themselves and all family members for a new family doctor at the https://patientconnect.nlchi.nl.ca/ website. It is fair to say you will be without a doctor in the next 3 months.

This has been an incredibly difficult decision for me. It has been a very challenging few years for both Yolanda & I and our families, and the time has come to make a change. It has been an incredible honour to know and care for so many of you over the last 17 years; I feel like I have become part of your families.

I appreciate your understanding and support, and wish you well in the future.

Fortunately, many family physicians are joining community Family Care Teams over the next year, and I am hopeful that people will still be able to access care without me. Registering for the Patient Connect NL website (above) will waitlist you for this service. You may also ask a family member if their physician can take you, but please be mindful that most physicians are already over capacity and their MOAs get dozens of requests daily.

I spoke with CBC's Terry Roberts for a few minutes yesterday, Remembrance Day, at the National War Memorial in St. John's. He asked what it meant to me personally to be there, and you can imagine I had a lot to say about my own time in service and my ongoing care for veterans. I knew they would condense our long chat into a 5 second snippet and this was exactly the sentiment I hoped he would air: that as much as I have loved caring for the veterans in my practice over the last 17 years, it is the additional privilege to care for their families that has touched me so much.

I come in at 8:50 for a dozen seconds, but the whole segment is just lovely.

Everyone is wondering why it takes over a month to get an appointment with me - so here's my short answer - patient needs have increased in number and complexity, almost doubling my previous requests - and there is still only one of me.

Since the pandemic began physicians all over the world have noticed a significant rise in people requesting assessment for adult ADHD and Autism disorders. We attribute this to the change in circumstances which caused people who were previously functioning ok to suddenly have difficulty coping; there has also been a huge increase in social media awareness of these disorders (which is great) and less stigma about coming forward (which is also great). Unfortunately, ADHD assessments and management take a lot of time in the first year, which I gladly give, but this takes from available time for other appointments. We BEG you, if you want to talk about possible ADHD, please make sure you tell Yolanda when booking, and she will make sure there is enough time and that you have done the homework before the appointment.

Otherwise there has been an absolutely deluge of people calling and emailing every day about mental health challenges, and it is more than I can manage with the urgency that people feel they need. Again, these assessments take a lot of time, and so my days are running increasingly further and further behind. I'm attaching the provincial websites for Mental Health resources and crisis resources below. Your family doctor should be helping with managing your chronic mental health concerns, but other professionals are available during a crisis.

The waitlist for almost all consultants and investigations has nearly doubled, so we are managing more complex medical concerns longer than before and functioning at an "almost specialist" level more than ever before. This means a lot of extra time gets spent researching and coming up with solutions to help while waiting to see the other specialists.

And then there is all the paperwork from schools, employers and insurance companies which has also doubled in recent years. Everyone needs "a note" for something, and they need it ASAP. Or their insurance company needs a form completed within 30 days - but there are already 50 similar forms waiting over a year. It's just all too much.

And so the normal family medicine concerns -acute infections, injuries, exacerbations of inflammatory disorders like asthma, Crohn's and arthritis, menstrual conditions, chronic medical conditions like hypertension, diabetes, liver, kidney, gastrointestinal, chronic pain, skin conditions etc are all getting crammed into a smaller available space, leaving almost no room for preventative health care (like mammograms, osteoporosis screening, infant, child and geriatric assessments).

Both Yolanda & I are working longer and harder than ever before, and just like a computer with too many windows open, that probably means we are being less effective. We want to thank the 99% of you who are understanding and patient when you have to wait for an appointment, or when your visit/call is 2 hours behind or gets rescheduled. We're honestly doing our best.

Yes, we recommend everyone get your influenza and COVID vaccinations on schedule just like Dr. Fitzgerald recommends. You can get them through your pharmacist or your public health nurses in the community, we simply don't have time to add them in.

"Dr Cooper will be unavailable for appointments from September 25th to 29th."

This blocked time each month is required to focus on over 120 requests for insurance forms, medication forms, legal letters, disability medicals and other administrative tasks which cannot be completed during clinic visits. In addition to consuming a great deal of clinic time* MCP will not pay physicians for their care** during an appointment if a third party form is requested during the same visit.*** Anything you request because your employer, school, coach/sport, lawyer, disability provider, insurance company needs is considered a non-insured service.**** Anything prescribed, such as any therapy, aids, accommodations, or recommendation as a consequence of my clinical assessment would be fine.

*Forms can take anywhere from 10 mins to 4 hours each
**Physicians are only paid for actual visits ($36-54 each depending on age) not time outside of the patient visit. MCP routinely audits charts and can refuse payment for services already provided. Physicians pay >60% of income for overhead and taxes. Most family physicians see between 18-30 visits a day and work 9-10 hours in clinic before heading home to complete paperwork afterhours.
***WorkplaceNL is the only exception to this.
****Non-insured services are usually linked to restricting insurance benefits that people without insurance don't require (ie. anyone can book a massage, you only need a note so you can get money back, or to get your time off work paid) or to offloading legal responsibility (fitness for work, sport and school) or employee management responsibility to the physician (all of those sick notes & accommodations make ME responsible if something happens to you). Taxpayers would be shocked to see how much of the country resources (money, energy and time) is consumed by visits which have no clinical relevancy.

A decade ago, this would take a half-day every other week and now it's a full week every month. In a recent survey, NL family doctors reported spending an average 43 hours a week seeing patients and over 10 hours every week on administrative tasks (ie. not seeing patients). Administrative time includes clinical tasks like charting, preparing referrals & reviewing results and also includes all of these other non-insured services. The majority of administrative time is done after-hours during personal/family time. Working over 50-hours a week, with hours of it not related to actual clinical needs, is driving family doctors to burn-out. It's getting more & more difficult to maintain the mental energy to meet these needs, especially when the majority of clinic visits are getting more & more complex.

All of this is why it takes a long time to get an appointment, and why requests are taking longer to be completed.

We sincerely regret all the delays and inconveniences caused by things completely out of our control.
