Important Notice: Closure of Dr. Devgan’s practice
I am writing to inform you that, after careful consideration, I have decided to close my medical practice effective May 31st 2025. This decision was not made lightly, and I deeply value the trust and relationships we have built over the last few years.
Unfortunately, I have not been able to find a replacement physician to take over my roster. As a result, I will need to discharge all patients. You do have the option to be put on a waitlist here in our clinic and if we are able to secure a physician by May 31st you will be contacted and offered to sign on with that doctor. Otherwise, you will remain on the wait list and may be contacted later if a physician becomes available.
You are also encouraged to sign on with Health Care Connect, a program that assists Ontarians in locating healthcare providers. Visit the Health Care Connect Website (https://hcc3.hcc.moh.gov.on.ca) or call 1-800-445-1822 for more information. I do realize that you will have to de-roster from my practice to complete the registration. I will continue to provide care for you until May 31st even if you are derostered unless you request to have your care transferred to a new family physician.
If you are on chronic medications, please have your pharmacy fax me a list of your medications prior to May 14th so I can ensure you have enough renewals for 1 year. Alternatively, you can book an appointment to review your medications with me and have them renewed that way.
Any results that come in prior to May 31st will be followed up by myself. For any imaging results or investigations that come in after the date of closure there are some options for you to access your results and find appropriate follow up. Connect My Health will give you access to your imaging and labs that were done in participating hospitals. More information can be found at https://info.connectmyhealth.ca/. Both Dynacare and Lifelabs have patient portals available to you at a cost. Only bloodwork collected at their clinics will be reported on their portal. Please ensure you obtain these results and arrange follow-up with another healthcare provider, as I will not be able to manage follow-ups after the closure date.
Your medical records will remain securely stored at this clinic. You can request a copy for your own records or have them transferred to your new healthcare provider. To arrange this, please contact the clinic at 519 245 3590 or have your new healthcare provider fax a request for the records on your behalf. There is an administrative cost of $25.00 as well as the OMA rate of $30 for the first 20 pages and $0.25 for each page thereafter per chart.
It has been an honor and a privilege to serve as your family physician. I am deeply grateful for the trust you have placed in me and for the relationships we have developed over the years. I wish you and your family continued health and happiness.
