CMA JOURNAL CLUB

Yesterday I was a cohost with .runels.md for his Cellular Medicine Association (CMA) Journal Club. It was quite an honor to be his invited guest.

We talked about PRP alopecia treatment for trans patients, interstitial cystitis, and a meta analysis publication supporting the O-Shot.

Then I took over with an abridged lecture on male enhancement using PRP, neurotoxin, and fillers. It was a fun interactive experience. The finalized version should be airing in the next few days. Stay tuned!!!

SCULPTRA

These are the Before and After photos of our patient’s Sculptra treatment. She received a total of 4 vials over the course of 2 months. These photos are 4 months after her first treatment and 2 months after her second.

We love the restored volume in her face and skin glow. Her features are much softer and more youthful without looking “done”.

SCULPTRA is an FDA/HEALTH CANADA-approved injectable that helps gradually replace lost collagen—the most common protein in the body that is used to form a framework to support cells and tissue—for results that can last up to two years.

I prefer the technique of injecting Sculptra deep on top of bone to help revitalize collagen production to restore structure and volume and to improve the skin’s integrity as there is communication between deep and superficial fibroblasts.

As the Sculptra PLLA microparticles are absorbed, they help stimulate collagen production. Rebuilt collagen strands begin gradually helping to restore facial volume and the look of fullness to wrinkles and folds.

One vial of Sculptra is recommended per decade of life. 2 vials are administered in one session and repeated every 4-6 weeks. Maintenance is recommended every 10-12 months.

ONE & DONE PRP HAIR REGROWTH TREATMENT

We offer 2 great treatment options to restore hair density: with and without needles. Some prefer injection using their own blood as seen here. We’ve eliminated the necessity for a series of 3 treatments by delivering an abundance of therapy in 1 sitting.

Platelet Rich Plasma (PRP) Therapy utilizes the power of highly concentrated platelets that contain growth factors which are responsible for healing and repair. Platelets also contain cytokines that draw stem cells to the area of injection. In addition, platelets when activated (i.e. release their contents) signal the body to create new blood vessels to improve circulation. We draw 120mL of blood compared to most clinics that only draw 10 - 20mL. These blood draw volumes significantly impact efficacy. Obviously the more platelets that are injected the more therapeutic the treatment. Hence, PRP must be rich in platelets.

CONGRATULATIONS TO NURSE GLORIA ARIAS FOR COMPLETING THE RARE MALE ENHANCEMENT DOUBLEHEADER.

Nurse Arias went from the west to the east from a heat advisory to a winter advisory from Beverly Hills to Toronto to obtain a full scope and understanding of male enhancement. Nurse Arias was already registered with my course yesterday, but she was able to attend Male augmentation cadaver workshop last week moderated by and myself.

Nurse Arias completed a very challenging augmentation procedure on an uncircumcised previously augmented patient. He had contour irregularities which did not bother him. However we were able to sculpt his final results to a more natural appearance all the while enhancing his size. Our patient was so happy, but then again they usually are.

APTOS PLLA/MCA THREAD LIFT

Here are the before and immediately after photos of our patient’s Aptos Thread Lift. As you can see, her face looks lifted without adding volume from fillers. Her face looks less tired as her nasolabial folds and marionette lines have softened. We have supported her lower face by anchoring the suspension sutures to her immobile areas by her lateral cheek.

Aptos is Health Canada approved and delivering WOW results immediately and typically lasts 18mos to 2yrs. One treatment delivers close to 1 meter of suspension sutures equivalent to 4 vials of Sculptra. They come in longer lengths such that you can make 2 to 3 passes with the same individual cannula.

These features allow for more lifting capability. There is no need to tie suture knots. They are anchored between additional passes of the cannula. They require a surgical field and sterile technique which allow the provider to actually touch the cannula closer to the entry site allowing for better control.

I was one of the first providers in Canada to use PDO threads. I started January 2018 but discontinued in 2021. Results were hit or miss. Longevity of the results were less than expected. No PDO threads have ever been approved by Health Canada. For these reasons, I discontinued. I’m finding Aptos threads to be far superior to the PDO threads I had access to years ago. My colleagues in the US, however, are doing great work with new generation PDO threads but they are unavailable in Canada.

CALF-TOX: NEUROMODULATOR CALF SLIMMING

Neuromodulators like Botox, Xeomin, and Dysport are commonly used as anti-wrinkle treatment. They can also be used for calf slimming. In this application we inject the medication into bulky muscles causing them to relax. This has a slimming effect.

The key to success is appropriate dosing injected deep into the belly of the muscles. A slow and steady approach is prudent here as calf muscles serve in ambulation and posture. Start conservatively and redose as needed on follow up visits.

Dosing will vary from person to person as initial presentation varies in severity and may affect final outcome.

GETTING MY GLOW IN BEVERLY HILLS

I’m speaking at tomorrow and want my skin to glow.
