As part of our Best Practice Spotlight Organization (BPSO) journey, we continue to advance clinical excellence through the implementation of Best Practice Guidelines, including the Assessment and Management of Pressure Injuries for the Interprofessional Team.
One of the topics in our recent Nursing Clinical Skills Day focused on offloading, turning, repositioning, and selecting appropriate support surfaces- best practices for preventing and reducing pressure injuries.
We are proud of our teams for their ongoing commitment to evidence-based practice, continuous learning, and delivering exceptional care.

The Toronto Grace Remote Care Monitoring (RCM) program continues to make an impact across Canada.
Now active in all five regions of Ontario — East, West, Central, North, and Toronto — and with projects in PEI and BC, the program supports thousands of clients each month through real-time, community-based monitoring.
Partnering with hospitals (including Hospital programs), primary care teams, Ontario Health atHome, and local paramedic and police services, the RCM program provides integrated, at-home care that helps older adults living with frailty safely transition from hospital to home.
Delivering care that’s connected, compassionate, and close to home. ❤️

We’re so pleased to welcome Major Pauline Gruer-Caulfield, our new Director of Spiritual & Religious Care, to the Toronto Grace Health Centre family.

In her own words:
“It’s a privilege to be entrusted with this role. As you know, our chaplaincy team is here not only for patients, but for each of you as well—please feel free to reach out at any time.”

Pauline also shared a deeply personal connection to The Grace:

“Earlier this year, my father spent his final days on the 6th floor. The compassion, dignity, and respect shown by every staff member allowed me to simply be a daughter again. That experience showed me what Grace care truly means.”

Nursing Skills Training Day at The Grace!

Our new nurses took part in Nursing Skills Training Day — a hands-on learning experience designed to strengthen foundational clinical skills and build confidence in care delivery.

Throughout the day, participants rotated through six interactive skill stations, practicing essential techniques such as:
�💉 Intravenous infusions�
🌬️ Oxygen therapy
�🩹 Pressure Injury Prevention�
🥤 Enteral feeding�
🧪 Tube insertions
ECG Monitoring

It was an inspiring day of learning, teamwork, and professional growth — all part of our ongoing commitment to excellence in patient care.

Patient Story: Nicholas’s Journey of Strength

Nicholas, 42, has faced lifelong health challenges — including major hypotonia, a heart transplant, two kidney transplants, and serious respiratory issues. After months on a ventilator, he was admitted to Toronto Grace’s Recover/Vent Unit in July 2024.

With the support of our Chronic Critical Illness team, Nicholas regained strength and reduced his need for ventilation. Through the Transition Home Program, his mother Angela learned to manage his care, and Nicholas was discharged just in time for Christmas.
