Crooked noses are always a challenge in rhinoplasty. We can usually get significant improvement but not perfectly straight. Our amazing patient was kind enough to share her results 4 months after combined structure preservation rhinoplasty to straighten the nose, reduce the bump and refine the tip.

Our amazing patient was kind enough to share her results 4 months after revision rhinoplasty. As is often the case with revision rhinoplasty, the nose was rebalanced to make it look more natural and proportionate. The upper third was built up, the lower bridge was reduced . The nose was straightened and the breathing improved.

You can see that the tip rotation and projection has stayed about the same over time. The main change that occurs after 4 months is further refinement of the tip. This time course for this will vary between patients depending on skin type and healing properties.

This was a tough case. Severe trauma with multiple fractures leading to very asymmetric and deviated nasal bones. The piezo or ultrasonic rhinoplasty was very useful in shaping the nasal bones precisely. These types of noses will never be perfectly straight or symmetric but we can usually get significant improvement. The breathing is almost always impacted as well and improving the function is often the thing most appreciated by our patients.

Refinement rhinoplasty, the bridge was reduced and the tip and nostrils were narrowed and the overall nose was straightened.

On the table photos before and after revision rhinoplasty. She had collapse of the middle third of the nose, rounding above the tip (polybeak), droopy tip, crookedness and breathing issues. Revision surgery like this typically requires cartilage to rebuild the nose. We sometimes get that cartilage from the nasal septum if available, ears or rib. We also used temporalis fascia taken from an incision hidden in the hair. This allow some camouflage and filling effect in thin skin. The pale bluish discolouration you see is common in thin skinned patients undergoing revision surgery. It will go away very soon after surgery. Revision rhinoplasty is challenging and should never be taken lightly. In general while we always strive for perfection we usually get improvement and a more natural looking better functioning nose . You have to weigh the risks vs the benefits.
