When we think of sciatica, we often think about the sciatic nerve being injured or irritated.

In reality, most of the time “sciatica” comes from a disc herniation in the lower spine, which can irritate and sometimes compress a NERVE ROOT.

Think of a nerve root like a tree trunk that comes out of the spinal cord, eventually giving rise to the branches, sticks, and twigs (rest of the nerves).

The nerve root may give rise to the sciatic nerve, as well as many others, so it’s a bit misleading to make it all about the sciatic nerve.

I’ve seen harmful statements ruin the lives of patients. It’s not always this bad, but it truly can be.

We have to be better communicators.

One of the most frustrating things about all of this is that many nocebic statements don’t need to be said at all.

Sure, sometimes we have to share bad news with a patient. We can always improve in our delivery here.

But so many negative things are said with such confidence and 100% certainty when they shouldn’t be said at all.

What value is it to say to someone that their x-ray is the worst they’ve ever seen, even if it’s true?

How are we still perpetuating myths about the dangers of asymmetry, moving wrong, muscle imbalances, or the all-or-nothing dangers of certain activities?

We should strive to be better communicators, more optimistic, and aim to question and evaluate our own beliefs so that we can stop perpetuating misinformation.

ACL patients are often told that they MUST get a surgery if they want to return to sport.

This is true for some - sometimes surgery is the best option. But it wasn’t true for Viven and for many others who have successfully rehabbed without surgery.

Being told that you HAVE to get the surgery and that conservative treatment is not an option is incredibly unfair.

ACL surgeries are second traumas to the knee that don’t come without complications. And they don’t have perfect success rates.

For a lot of people, trying conservative rehab and making a delayed decision to get surgery if non-surgical rehab fails makes a lot of sense.

Spread the word. People should know that this non-surgical rehab is an option.

While it is possible to do harm by doing too much, the majority of health issues that burden our society are associated with lower amounts of physical activity.

When health care workers and trainers demonize certain movements or types of exercise, they may create fear that acts as a barrier for people to become more active.

A lot of the ideas around avoiding certain exercises are not grounded in evidence but are still widely believed.

The result is unhelpful and even harmful false information being perpetuated to the masses, which is really unfortunate.

It’s hard enough to get people moving. Let’s not make it harder. Let’s promote movement optimism.

30 minute leg workout. First BJJ class in a long time came a few hours after this, so I kept it short.

1. Single leg hops.
Do we lose the ability to jump because we get old or because we stop jumping? Probably both, but most of us stop and lose the ability prematurely.

2. Deadlifts. I started using straps and it’s a game changer. Angus Bradley only needed to say it 100 times in his posts before I listened.

3. Smith machine calf raises. The smith machine is massively underrated and over-hated. The built in stability really helps you just focus on the muscle. Great for calf raises.

4. Band assisted smith machine sissy squats (pictured first). My first set I only got 4 reps, so I wanted to make it a bit easier and I added the band. It worked better than I thought it would and I think this is a fun exercise that I’d recommend to almost nobody I work with 😂

One of the biggest takeaways is that many of us don’t use the biopsychosoical appropriately.

We talk about the enactive approach to pain, an updated model for making sense of pain.
