MedError Report a Medication Incident:

Taking or receiving a wrong medicine, or the wrong dose of a medicine, could harm your health.

MedError.ca, is part of the Canadian Medication Incident Reporting and Prevention System (CMIRPS), a Canadian initiative to prevent harmful medication incidents. The fact is, mistakes with medicines happen, but they are preventable. Mistakes with medicine are called medication incidents. We believe that consumers and health professionals can play active roles in preventing mistakes in health care. When incidents are reported and reviewed, problems that could increase the chance of an error happening can often be found and corrected. The MedError.ca site provides a convenient way for consumers to report medication incidents and to get information about using medicines safely. At the Institute for Safe Medication Practices Canada, our job is to study the incidents reported by you and recommend ways to prevent them. The more incident reports we receive, the better we can work together to prevent harmful medication errors from happening in the future. MedError.ca is funded by Health Canada and is part of a Canadian initiative to prevent harmful medication incidents, the Canadian Medication Incident Reporting and Prevention System (CMIRPS). MedError.ca provides a better way for consumers to report medication incidents and to get information about using medication safely.

❗Do you take MAR-Amlodipine 5 mg tablets? Health Canada has issued an advisory because some bottles may contain the wrong tablets.

The correct tablets are off-white and have 8 sides. The wrong tablets are round and white. See the image below.

If your bottle contains any unusual tablets or if you are unsure, return it to your pharmacy.

If you are unable to get a replacement immediately, continue taking your medication as directed but do not take the round tablets.

Contact your health care professional or call 911 if you are dizzy, have unusually high blood pressure or slow heartbeats.

Seek immediate medical attention if you have chest pain, a sudden headache, an inability to move or feel a part of your body, or impaired speech.

Residents and families have an important role to play in medication safety in long-term care homes. Devora Greenspon was a fearless advocate for elevating the voice of residents and was an active member of Ontario Association of Residents' Councils. For several years, Devora has been part of ISMP Canada's Strengthening Medication Safety in LTC Homes initiative in Ontario. She sat on our advisory committee, and gave us her time and wisdom to help us support staff and residents in improving in Ontario LTC homes. Devora passed away earlier this month and we are grateful to have had her expertise as part of our team.
