Just wanted to share a success story!
This patient came in with a new wound on his R/Charcot foot which started on the plantar arch and blistered on the lateral aspect of the arch onto the dorsal lateral foot.
Cause: Biomechanical as most Diabetic wounds are ! ie . he was trying his new work boots
Treatment:
Short Term : Culture and Sensitivity, generic broad spectrum ab x 2 weeks , Wound debridement, offload ! offload ! and more offload! (Darco boot)
Dressing of choice and also my most fav dressing of ALL time for wounds that dont need packing/no depth = INADINE gauze.
change Q2D.
Midterm : 1 week later
Follow up in 7 days (picture you see in the "after")
More Debridement was completed (not shown in the picture) .
Addressed his biomechanical fault and spoke to his pedorthis to modify his safety shoes.
C & S shows Staph so so need to change his ab . Continue Ab until finish
Long Term :
moving forward , patient education (continuous! and indefinite! ) in terms of offloading , pressure , types of shoes, orthotics, diabetic control, etc
In addition , I would like to add this patient was a wonderful patient with great BG control and overall health! The willingness of a patient is what makes the outcome so wonderful!
I will see him in a week and I am sure his wound will be fully healed ! I can share the updates once I see him .
