For 50 years, care at SRCHC has been about more than services — it’s about people feeling safe, welcomed, and connected.

At our Moss Park Consumption and Treatment Services site, that’s been meeting people where they are and building trust through human connection, understanding, and compassionate care.

Lamia Nouman, a community health worker shared this dream for the future:

“I hope people will still see SRCHC as a place that leads with a holistic, relational model of care — care that’s open-minded, low-barrier, and accessible for everyone who needs it, so no one is left behind.”

As public funding for this site comes to an end, we are determined to carry forward the impact of this approach.

Last week, South Riverdale Community Health Centre (SRCHC) held our Annual Staff Appreciation event, at the Evergreen Brick Works, to celebrate staff and their years of service at the organization.

Staff had the opportunity to collaborate with others in fun activities, such as shelter building, making seed balls, and going on nature walks. In addition, Service Awards were provided to staff that played a long-term role in providing care and service to clients at SRCHC.

South Riverdale Community Health Centre is creating a Community Advisory Committee to help shape our anniversary celebrations — and the future we’re building together.

We’re looking for 10 community members who:
· Live in the community
· Use our services or programs
· Are connected through local community organizations or networks

What started as a bold idea quickly became something real: a community-driven space built around people, their stories, and their hopes for a healthier future.

'50 Years · 50 Faces' celebrates the people who bring SRCHC to life. Together, their stories reflect where we’ve been, what we care about, and where we’re headed next. We’ve now posted ten profiles and will continue adding new faces and stories throughout the year, building toward 50 voices that reflect the richness and diversity of our community.

This Nursing Week, we’re celebrating the incredible nurses who provide compassionate, community-based care to people navigating substance use and mental health challenges.

Erin Telegdi, a nurse of 13 years and our Manager of Advanced Clinical Practice for Substance Use & Mental Health, shares her dream:

“My dream is that we will once again be funded to provide supervised consumption services as part of the full spectrum of substance use care. The Moss Park OPS/CTS has taught me that healing, for all of us, happens in community.”

As part of our 50th anniversary and series, we honour this vision for the future — while also facing a difficult reality. Our Moss Park CTS is set to close in June 2026 following the loss of public funding. Read our statement to learn more: https://bit.ly/4f9ye17

As we celebrate 50 years of care rooted in community, partnership, and connection, we are also honouring International Day of the Midwife (IDM), and the vital role midwives play in shaping more equitable, compassionate care.

We’re highlighting the MATCH program (Midwifery and Toronto Community Health) and the collaboration between SRCHC and to support care at the Early Pregnancy Clinic. When community and hospital teams work together, access improves, gaps are reduced, and people receive care with dignity at every stage of their journey.

Today, Shezeen Suleman — one of six midwives at SRCHC and co-lead of the MATCH program — shares her dream for the future of care:

“My dream is for more people to access safe, respectful, life-saving care and to create a different experience for them, one that truly honours people’s right to make decisions over their own body.”

As we look forward, we’re reminded that too many people still face barriers to care. Midwives work to ensure that every person has a voice, choice, and the support they need.

In honour of , we’re celebrating and thanking all of our physicians for the care, compassion, and leadership they bring to our community — including Dr. Angela Wong, our physician lead, who is sharing this week’s dream.

“My dream is for care to see the whole person — their medical needs, and their housing, food, and social connections. And to care for SRCHC staff, so we can continue to bring this dream forward every day and keep caring for our community.”

As we mark our 50th anniversary, we’re with our staff and community — deepening our commitment to care, connection, and community, every day. Together, we strive to show up for people in all the ways they need, while caring for the teams who make that care possible.

If SRCHC is able to remain inclusive and welcoming, caring for the whole person, not just the illness, it will continue to build a strong, healthy community for the next 50 years."

Shared by our very first patient, who continues as a patient and valued community member, today.

For 50 years, we’ve believed that health care includes treatment, having a safe place to live, access to food, meaningful connections, and a sense of belonging. This belief has brought staff, volunteers, partners, and community members together to build care that truly supports people’s lives.
