Humber River Health is proud to be part of the global conversation on advancing care.
From Birmingham, Beatrise Edelstein, our Vice President, Post Acute Care and Health System Partnerships, alongside Kathleen Kirk, our Clinical Program Manager, Schulich Family Medicine Teaching Unit & Integrated Care Services, recently represented Humber River Health at the International Conference on Integrated Care (ICIC26).
Their presentation, “Integrated Acute and Long-Term Care Through a Nurse-Led Outreach Model: Advancing Staff Capacity and System Collaboration,” highlighted the impact of our Nurse-Led Outreach Team ( ). Through mobile clinical support, IV therapy, capacity building, and seamless hospital-to-home transitions, this model continues to strengthen long-term care staff capacity while improving patient outcomes and positively impacting health care system.
They also presented the Lower Limb Preservation (LLP) Pathway which was co-designed by HRH’s SCOPE and LTC+ programs in partnership with Own Health’s vascular surgeons, Dr. Asem Saleh and Dr. Kerry Graybiel. This pathway was implemented to strengthen care in underserved communities across Northwestern Toronto. This community-based, equity-driven model focuses on preventing limb loss through integrated vascular care, early intervention, and coordinated support across hospital, primary care, and long-term care sectors.
