Canadian Forces Health Services (CFHS) medical and dental clinics will undergo an on-site assessment by an external non-government organization, Accreditation Canada, in May and June 2022. This assessment, referred to as “accreditation”, is a process that examines and validates all aspects of care delivery in a healthcare organization against national standards of excellence. It is a continuous process that CFHS is committed to participating in to ensure we stay up to date with improvements in the way health care is delivered and to confirm we are providing safe, high quality medical and dental care to the members of the Canadian Armed Forces (CAF).

24 CF H Svsc C and 1 DU Det Trenton will undergo the accreditation process 19 and 20 May. To contribute to the process, patients are asked to complete the Patient Care Feedback Tool (https://survey-sondage.forces.gc.ca/snapwebhost/s.asp?k=164321825644) which enables members to answer an on-line survey regarding the care they received.

1. effective January 4, 2022, Ontario will begin requiring the use of the enhanced vaccine certificate with QR code and the Verify Ontario app in settings where proof of vaccination is required.
2. Starting on December 20, 2021, Ontario will also begin requiring proof of vaccination for youth aged 12 to 17 years of age participating in organized sports at recreational facilities.

PROCESS FOR REQUESTING PRESCRIPTION REFILLS REQUIRED DURING HOLIDAY BLOCK LEAVE

1. The 24 CF H Svcs C Pharmacy will be closed from 18 December 2021 to 9 January 2022 inclusive and the pharmacy staff are currently preparing for the holiday block leave closure. All patients are requested to review their medications to ensure an adequate supply to last until 12 January 2022.

2. If your medication supply will run out between 15 December 2021 and 12 January 2022, request your refill no later than 6 December 2021 and indicate that it is a holiday supply request by calling the refill line at extension 2515, or by emailing your request to +Pharmacy@24 CF H Svcs C@Trenton (pharm@intern.mil.ca).

3. Medications requested this way will be available for pick-up from the 24 CF H Svcs C pharmacy from 13 to 17 December 2021; do not call to inquire if it is ready for pick-up. Patients will not be contacted by pharmacy staff unless there are concerns.

4. If you require a regular refill prior to 15 December 2021, please continue to use the regular process of providing 2 to 7 full business days’ notice by calling the refill line at extension 2515 or emailing +Pharmacy@24 CF H Svcs C@Trenton (pharm@intern.mil.ca), listing all medications required and include the date you plan to pick-up the medication.

5. Between 18 December 2021 until 9 January 2022 inclusive, please proceed to a civilian pharmacy of your choice for any new prescriptions and present your Blue Cross card for payment. Medications not covered by Blue Cross are not always reimbursed. Please request the Pharmacist contact the provider for a medication included on the CAF Drug Benefit List . The CAF Drug Benefit List is also available through a simple Google search on any computer. If the civilian pharmacy is not able to use your Blue Cross for your prescription costs and you have to pay out of pocket, please keep the official pharmacy receipt and submit it to the 24 CF H Svcs C Orderly Room (attn: Blue Cross rep) for reimbursement once we reopen in the new year.

6. Please note that during the Wing pharmacy closure, prescription refills cannot be transferred to a civilian pharmacy to be filled as regulations require the Wing Pharmacist to be present to complete the transfer. Please ensure all medications required during the holiday block leave are picked up prior to the closure on 18 Dec 21.

7. If you have any questions or concerns WRT to your medications, please contact any of our Pharmacists at extension 2515.

CAF members,

The new national proof of vaccination credential (PVC) has been developed by the Public Health Agency of Canada (PHAC) in partnership with jurisdictions across the country, including the Canadian Armed Forces (CAF). This digital PVC is a proof of vaccination that meets national and international standards and will make it easier for CAF members to prove their vaccination status during international travel and when returning to Canada. It can also be used wherever a proof of vaccination is needed, such as to access non-essential services where vaccination is necessary.

The CAF PVC will be issued to vaccinated CAF members. Others for whom the CAF maintained vaccination records, such as many of the dependents accompanying CAF members outside of Canada, may also request it.

The paper documents issued at the clinic at the time of vaccination remain valid proofs of vaccination.

Distribution to CAF members will start the week of 26 October 2021 and will be automatically sent to members’ DWAN and ECN email addresses. You may receive your PVC on a different date than your colleagues; distribution to all CAF members can take up to a week. The PVC will be automatically generated and sent to you if your medical record indicates that you have received COVID-19 vaccination(s) from the CAF (or registered your vaccination with your supporting Canadian Forces Health Services (CFHS) health clinic). After you receive the PVC by email, you can forward it to your personal email address so it can be kept on your phone, or it can be printed as needed.

If you don’t have access to DWAN or ECN, the PVC will be automatically created and kept as a file in your medical record. A centralized email address to request your PVC for those without DWAN or ECN will be announced soon. You may contact your supporting CFHS health clinic if you have questions.

If you received your vaccine(s) from another provider, such as a civilian health care facility, you must bring that information to a CFHS health clinic so that it can be entered in your health record; otherwise, a valid PVC cannot be created for you. Your CFHS health clinic may contact the outside provider to verify the details in order to ensure your vaccine record is accurate.

I encourage you to speak with your chain of command if you have any questions or concerns. The COVID-19 vaccines for Defence Team members webpage will also be updated with the most current information

Les documents papier remis par la clinique au moment de la vaccination demeurent une preuve vaccinale valide.

La distribution de cette preuve aux membres des FAC commence la semaine du 26 octobre 2021 et celle-ci sera envoyĂ©e directement aux adresses courriel de l’ECN et du RED des militaires. Vous pouvez recevoir votre code QR Ă  une date diffĂ©rente de celle de vos collĂšgues ; la distribution Ă  tous les membres des FAC peut prendre jusqu'Ă  une semaine. La preuve de vaccination sera automatiquement gĂ©nĂ©rĂ©e et envoyĂ©e si votre dossier mĂ©dical indique que vous avez reçu un ou des vaccins contre la COVID 19 administrĂ©s par les FAC (ou si vous avez inscrit vos vaccins auprĂšs de votre clinique de santĂ© locale des Services de santĂ© des Forces canadiennes [Svc S FC]). Une fois votre preuve reçue par courriel, vous pourrez la transmettre Ă  votre adresse personnelle pour la conserver sur votre tĂ©lĂ©phone ou tablette. Il vous sera aussi possible de l’imprimer et utiliser une version papier.

Si vous n’avez pas accĂšs Ă  votre compte du RED ou de l’ECN, un fichier de preuve de vaccination sera automatiquement créé et versĂ© dans votre dossier mĂ©dical. De plus, une adresse Ă©lectronique centralisĂ©e sera annoncĂ©e sous peu et permettra aux gens n’ayant pas accĂšs aux comptes du RED et de l’ECN de demander leur preuve de vaccination. Vous pouvez communiquer avec votre clinique de santĂ© des Svc S FC si vous avez des questions.
Si vous avez obtenu votre ou vos doses de vaccin auprĂšs d’un autre fournisseur, comme un Ă©tablissement civil de soins de santĂ©, vous devez transmettre cette information Ă  une clinique de santĂ© des Svc S FC afin qu’elle puisse ĂȘtre consignĂ©e Ă  votre dossier mĂ©dical; une preuve de vaccination valide ne pourra ĂȘtre créée pour vous autrement. Votre clinique de santĂ© des Svc S FC pourrait communiquer avec ce fournisseur externe afin de vĂ©rifier que les renseignements figurant Ă  votre dossier de vaccination sont exacts.
