Accessory and lateral canals

Accessory and lateral canals are small offshoots of the main root canal and can play a significant role in endodontic outcomes, especially in persistent infections and healing after treatment. The prevalence of these canals varies by tooth type, root location, and method of detection (e.g., histology, CBCT, clearing techniques). Apical third is the common area (75–90% of accessory canals found here). However, they can present in the middle third,cervical third, and furcation (in 30–40% of molars). Biological cleaning with irrigants and laser assisted irrigation may help manage them.

Root Canal on the Upper Second Premolars: Easier Said Than Done!

The patient presented with a painful swelling (bump) in the buccal vestibule.The pain and swelling had started abruptly one week prior, without any known initiating factor.

The periapical radiograph showed an adequate previous root canal with a large periapical lesion. The CBCT scan revealed a missed palatal canal ( black arrows).

Although the root canal treatment appeared technically adequate, the missed palatal canal allowed for acute infection. The large periapical lesion and acute symptoms highlight the critical importance of identifying all canals, particularly in upper second premolars, where palatal canals are frequently overlooked.
