PCN Clinical Pharmacist

The position offers a hybrid work option - 2 days working from home and 3 days in Mission split between the Mission Community Health Center and neighbouring physician medical clinics. No weekends, STATS and evenings!

The successful candidate will enjoy being able to work in a collaborative team alongside other allied health professionals and prescribers in the provision of direct patient care through optimization of medication therapies, chronic disease management, drug information and education. Self-motivation, excellent communication skills, a strong clinical background, and adaptability to process improvements are all key assets to success in these positions.
