True story

Ok I embellished the Malaria and Ebola bit

But this was a real story of a real patient who presented with significant hot flashes, va**nal dryness and recurrent urinary treats infections

She also has an elevated BMI, sleep apnea, was prediabetic and had borderline high blood pressure. All of these increase ris for frequency and severity of VMS (vasomotor symptoms - hot flashes and night sweats).

Instead of estrogen, she was referred to internal medicine, rheumatology and infectious disease - because that’s a great use of publicly funded healthcare

She came to see me about her “fevers” saying she had multiple fevers in a day and had to track her temperature to see how high they were getting

I gave her estrogen and her “fevers” resolved within 3 days

She cancelled her infectious disease consult

Estrogen is standard of care for VMS.

Estrogen is standard of care for va**na dryness.

Vaginal dryness and loss of estrogen increases risk and susceptibility for UTIs.

Estrogen is standard of care.

Funny / not funny

So many women have a version of this story

My patient being told “it’s too pink and young looking to be in menopause” - even though she was experiencing dryness and discomfort

Mine is being left while a Page (remember pagers?!) was answered about stocks crashing 🤑

HERE’S WHAT YOU NEED TO KNOW

You have the right to have someone else in the room

Communication should be neutral and professional, “the tissue looks healthy, but I understand I might not be seeing what you experience intermittently throughout your cycle”

These exams are sensitive and should be respectful

Unfortunately most inappropriate comments and treatments will not meet the threshold for the complaint process (I have inquired)

So the assertiveness and advocacy starts with YOU:

“I don’t / didn’t appreciate that”

“This made me uncomfortable”

“I would like to discuss your impressions. What is your clinical opinion?”

If it’s hard to speak up (of course it is) - putting your concerns in writing is valid and will ensure they are filed

You deserve the best care

Standup for you, stand up for others

Let’s demand ACCOUNTABILITY from the ground up because it’s not coming any time soon from the top down

In case you were wondering…

IV iron is standard of care for patients who have severe iron deficiency anemia, those who do not respond to oral iron therapy and those who have conditions of malabsorption (Celiac, Crohn’s, Colitis, Post-Bariatric Surgery)

Unresponsive to oral supplementation is qualified by >30 point increase in your ferritin over 3 months, or >10 point increase per month.

Unresponsiveness may also be qualified by intolerance to oral iron supplementation - or when the side effects are too significant and disruptive to continue oral iron

IV iron is safe and effective when administered appropriately and when clinically indicated (which is way more than it’s currently being offered)

I’ve administered almost 100 IV iron treatments and can’t wait to see that number multiply over time - because too many women are tired and we need you in your power

This is about so much more than iron deficiency - but while we’re on the topic

Iron deficiency increases your risk for ADHD by almost 4x

Heavy menstrual bleeding should be investigated with imaging and you should have your iron tested and effectively treated, if low

Persistent iron deficiency that doesn’t respond to oral supplementation often warrants Celiac testing

Indigenous and marginalized people are disproportionately affected by iron deficiency

Don’t even get me started on standards of care for menopause and hormone therapy

The standard of care should be to properly treat the 40% of menstruaters who are iron deficient and the 90% of pregnancies in which iron deficiency occurs

The standard of care should be to understand physiology

The standard of care should is to listen and believe you

Regulated medical practitioners willingly sign up to follow a set of rules and standards. Let’s hold each other accountable, regardless of our titles, our genders, our level of power, our earnings and our biases.

Because this is actually supposed to be about YOU, the patient.
