Care teams are able to configure thresholds for alerts and use the flagging system to identify .

The challenge

A major driver of the healthcare system burden for Cardiovascular patients is hospital activity, particularly when a patient’s condition deteriorates and they require a non-elective admission. Cardiovascular patients average length of stay is 50% higher than the average for all diagnoses (10 days vs. 5 days).

Our solution

The Docme solution develops a schedule for unscheduled care for patients with Heart Failure, Chest Pain, and Pre-eclampsia using digital enablement. Patients are able to report on their health status using the Docme App and Devices. Their data is transferred directly into the Care Information Exchange (via Patients Know Best) to be used by healthcare professionals .
