This 50 year old lady is of relatively darker underlying complexion (Fitzpatrick Type 4 to 5). Our major concern at the beginning was post-inflammatory hyperpigmentation (PIH). PIH is a
common phenomenon in individuals of darker skin involving reactive increase in
melanin production after skin treatments.
Treatment progression was cautious and slow. We took treatment breaks of two to three
weeks at a time to monitor stability.

Everyone thought she had some freckles on her cheek.
On careful inspection, however, it was apparent that the pigmentation was not superficial. It was likely deeper in the epidermis.
After laser removal of the top layer of pigmentation, the same hazy greyish coloration remained. It took a few further treatments to completely clear residual pigmentation at the deeper layers to arrive at this result.

This lady was at our clinic six years ago to have her melasma treated. At that time, the
melasma was initially complicated by PIH (post-inflammatory hyperpigmentation) before eventually responding well after a tortuous course.

She returned recently due to recurrence of her melasma. This time around, even though the melasma was visible, the overall skin condition was not as grave as it had been a few years back.
She shared that she had been diligent in maintaining the health of her skin. This is testament to the hypothesis that optimal skin health does serve to mitigate the severity of melasma.
This time around, exceptional precaution was taken to avoid running into PIH. Notably, her melasma problem resolved uneventfully over a much shorter course of treatment. We will continue the program for a few more sessions in the hope of achieving yet bigger improvements.

We encouraged continued diligence in skin care in the hope that her melasma will not
recur for many years to come.

For skin of darker tone, such as Fitzpatrick 4 or above, the biggest challenge of laser
treatment is rebound pigmentation or post-inflammatory hyperpigmentation.

Therefore, it is with utmost care and caution that we undertake to treat this East-Indian
lady’s long-standing pigmentation problem on her cheeks.

Treatment sessions are spaced at wide intervals to reduce over-stimulation and to
ensure stability of progress.

The photos show that we are achieving gradual improvement. Treatment is still on-going
as we endeavour to achieve eventual clearance of this lady’s pigmentation problem.

Mr. X did not believe he had any chance.
He thought he would be stuck with the discoloration on his face forever, as it had been there for years.
Fortunately, his wife thought otherwise, and brought him to us.
On examination, Mr. X had both superficial and deep-seated pigmentation on his face. He proceeded with treatment half-heartedly.
On completion, Mr. X was in both disbelief and regret; disbelief as to the amazing change, regret as to his hesitancy in not having come forward earlier.

A young Philippino man with a large birth mark at his under chin and upper neck.

After 5 laser treatments done over a three-month period, he was pleased with the improvement.
He was aware there was room for further improvement, but he would like to take a break for the time being.

Further treatment with two to three more sessions will likely bring about more change.

We have done a follow-up with the same young man with acne scar from 2025-January.
You will recall that he had extensive scarring especially at the forehead.
He was treated with 3 sessions of microneedle-radiofrequency, subcision and platelet-
rich-plasma.
In the April photos, with the elapse of another three months, we can see further
improvement in every respect of his skin as compared to the initial follow-up in January.
The caveat is that progressive improvement continues for many months after treatment
is completed.

This old young man is his twenties and has extensive acne scars, especially at the forehead.
Some prominent features were atrophic scarring, marked unevenness, diffuse red
discoloration and stubborn skin-tethering adhesions.
Treated with 3 sessions of microneedle-radiofrequency, subcision and platelet-rich-
plasma over a period of four months.
Much improvement can be seen on the after picture at 2 months post-treatment.
Further improvement is expected to take place over the next few months.

Dr. Yip saw Ms. C 8 years back for her melasma. At that time, the treatment was quite straight forward, and the result was readily forthcoming.

Ms. C returned recently for the same problem. However, the melasma had become more intense and extensive.

As usual, Dr. Yip cleared out much of Ms. C β€˜s melasma, albeit it took much greater effort this time around. The treatment regime included laser application innovations that Dr. Yip had just developed in recent years.

On after-thoughts, Dr. Yip exclaimed that if Ms. C had presented her melasma 8 years ago with her present day’s more difficult melasma features, the treatment techniques used in those days would not have been adequate to address her more complicated
melasma of today.

That’s so much the treatment of melasma has evolved since Ms. C last visited in 2016.
