🔍 Surgical Equity Milestones: A History Lesson

In 2015, the G4 Alliance was founded to advocate for neglected surgical, obstetric, trauma, and anesthesia (SOTA) care worldwide. Representing over 70 organizations across 160+ countries, the Alliance champions access to safe, timely, and affordable surgical care as a global health priority.

By uniting stakeholders from civil society, academia, and healthcare, the G4 Alliance amplifies the voices of underserved communities and drives policy change—laying the groundwork for a future where no one dies from lack of essential surgical care.

Dr. Hannah Wild and members of the Global Surgery Lab, have recently published the results of a study to evaluate the second iteration of the Vital Surgery Training Program (VitalSurg) a task-shifting initiative designed to build local surgical capacity in surgical deserts by training generalist doctors to perform essential procedures.

Implemented in partnership with Médecins Sans Frontières (MSF), the program has undergone 2 pilot iterations in South Sudan. The study team sought to examine the feasibility, effectiveness, and adaptability of competency-based surgical training embedded within humanitarian clinical care.

🔍 Surgical Equity Milestones: A History Lesson

In May 2015, the World Health Assembly passed Resolution WHA68.15 — the first global political commitment to recognize surgery and anesthesia as vital to Universal Health Coverage.

This landmark resolution elevated surgical care to a health systems priority, pushing countries—especially in low- and middle-income regions—to integrate surgical and anesthesia services into national plans. Backed by the WHO and global health leaders, WHA68.15 continues to shape policies and investments that bring life-saving surgical care closer to those who need it most.

🔍 Surgical Equity Milestones: A History Lesson

In 2014, the Lancet Commission on Global Surgery was launched to spotlight a critical truth: surgery is not a luxury, it's a necessity. This groundbreaking initiative reframed surgical care as a fundamental part of universal health coverage, not just a specialty service for the few.

With input from over 110 experts across 40 countries, the Commission introduced six core indicators to measure surgical system performance and called for national investment in safe, timely, and affordable surgical care. It laid the foundation for the global surgery movement and offered governments a roadmap for closing the surgical access gap.

A turning point in global health, the Lancet Commission continues to shape policy and advocacy efforts to this day.

🔍 Surgical Equity Milestones: A History Lesson

In 2008, the WHO launched the “Safe Surgery Saves Lives” campaign and introduced the Surgical Safety Checklist — a simple yet powerful 19-item tool to make surgery safer for everyone, everywhere.

Developed with input from Dr. Atul Gawande (check out his 'Faces of Global Surgery' feature on our page to learn more), this checklist drastically reduced surgical complications and deaths, especially in resource-limited hospitals. It quickly became a global standard, offering a low-cost, high-impact solution to improve surgical outcomes and advance health equity.

Now used in over 100 countries, the checklist remains a cornerstone of safe and inclusive surgical systems.

Is safe, high-quality surgical care possible in a conflict zone?

A new study says yes.

🇦🇫 Over 15 years, GSF partner EMERGENCY provided more than 120,000 surgeries and 695,000 outpatient visits across Afghanistan.

As conflict shifted, so did the care:

From war trauma to road accidents, from emergency surgery to maternal health.

This isn’t just a story of Afghanistan but demonstrates that sustained, high-volume surgical and obstetric care is feasible in conflict-affected settings.

âś… Surgical care systems are the most adaptable and can best address the needs of the most under-served communities.
