Building a Level 2 OHP clinic in Ontario is far more complex than a standard medical clinic buildout. These projects involve:

-procedural workflow planning
-recovery areas
-HVAC and infrastructure coordination
-infection prevention requirements
-CPSO accreditation readiness

One of the biggest misconceptions is that CPSO accreditation happens only at the end of the project. In reality, successful Level 2 OHP projects are planned around compliance from the very beginning — including:

-infrastructure rough-ins
-workflow separation
-IPAC considerations
-recovery planning
-policies, procedures, and SOP requirements

Mistakes discovered late in the project can become extremely expensive and may delay opening timelines significantly. This is why healthcare-specific experience matters.

Medical clinic construction is very different from standard commercial construction. Unfortunately, many of the most expensive mistakes are not discovered until after the clinic is already built and operational. We commonly see issues related to:

-poor plumbing planning
-inadequate HVAC systems
-inefficient clinic flow
-incorrect electrical capacity
-not planning for future growth

The problem is most of these mistakes are difficult and expensive to fix later. The clinics that perform best long-term are usually the ones designed around:

-workflow
-patient throughput
-operational efficiency
-future scalability

Healthcare construction is highly specialized — and small oversights early on can create major operational problems later.

Not all medical clinics are structured the same.
On the surface, many clinics can look attractive:

- low overhead
- nice space
- promises of patient volume

But behind the scenes, the wrong clinic structure can lead to:

- lower income
- more stress
- operational headaches
- poor long-term stability

Some of the biggest red flags we see include:

- overhead that seems too good to be true
- weak staffing support
- poor patient flow
- lack of billing oversight
- unclear leadership and management

The reality is a well-run clinic often matters far more than simply finding the lowest overhead percentage.

One of the first questions physicians ask when joining a clinic is:

“What’s the overhead?”

On the surface, lower always seems better.

But in practice, overhead is one of the most misunderstood parts of choosing the right clinic.

We’re seeing:

* most competitive clinics now in the 20% to 25% range
* stronger, established clinics closer to 25% to 30%
* anything below 20% often comes with trade-offs

The real question isn’t just the percentage. It’s:

* what support you’re getting
* how efficient the clinic is
* how many patients you can actually see
* and how much you’re taking home at the end of the day

We’ve seen physicians earn more in a well-run 25% clinic than in a poorly structured 15% one.

FHO structures are often misunderstood — especially when it comes to leadership roles, OPA funding, and GMLP. Most physicians focus on clinical work, but the reality is: A well-run FHO depends heavily on how the group is structured behind the scenes.

Key areas that often get overlooked:

*leadership responsibilities (lead vs co-leads)

* proper use and reporting of OPA funding

* how GMLP and Enhanced GMLP are handled internally

* governance and accountability across the group

We’ve seen that the difference between a smooth-running FHO and a struggling one usually comes down to structure, clarity, and ex*****on at the leadership level.

Physician Assistants are becoming a more visible part of healthcare delivery in Ontario.

But there is still a lot of confusion around:

* how they are regulated
* what they can and cannot do
* how they fit into a clinic setting

The key point is this:

PAs are not independent practitioners. They work under physician supervision, and their role is defined through delegation and clinic structure.

With the introduction of CPSO’s Physician Assistant Register, there is now more transparency and oversight — but the core model has not changed.

For clinics, the opportunity is in how PAs are integrated:

* supporting patient flow
* improving access
* increasing efficiency without increasing physician workload

One of the biggest decisions physicians and investors face is whether to buy an existing clinic or build one from scratch.

Both options can work — but they lead to very different outcomes.

Buying an existing clinic offers:

* immediate revenue
* an established patient base
* operational systems already in place

But it also comes with risks that aren’t always obvious upfront.

Starting from scratch takes more planning, but allows you to:

* design the clinic around your model
* structure partnerships (including pharmacy and allied health)
* build long-term value from day one

In many cases, the decision comes down to short-term convenience vs long-term control.

We broke it down here:

When most people think about the cost of running a medical clinic, they focus on rent and staffing.

In reality, those are just the starting point.

The actual cost structure of a clinic goes much deeper — including:

* technology and EMR systems
* medical supplies and equipment
* billing and operational oversight
* insurance and compliance
* inefficiencies that impact profitability

Many of these costs are not obvious upfront, but they play a major role in how a clinic performs over time.

Understanding the full picture is critical for physicians and investors looking to build or grow a sustainable clinic.

Attracting patients to a medical clinic in Ontario is not just about walk-in traffic.

While walk-ins can drive volume, they are not always the most reliable or sustainable way to grow a clinic long-term.

The clinics that perform best tend to focus on:

* building a strong family practice base
* integrating pharmacy and allied health services
* creating internal referral pathways
* improving access and patient experience

These factors create consistent, predictable patient flow — not just short-term spikes.

If you’re looking to grow your clinic in a more structured and sustainable way, it starts with how the clinic is set up and positioned.

For many physicians in Ontario, increasing income often feels tied to working more hours or seeing more patients.

In reality, a lot of revenue is lost through inefficiencies in billing.

Across FFS, FHG, and FHO models, we consistently see missed opportunities such as:

* incomplete billing
* missed premiums
* unaddressed rejections
* outdated billing habits

With the recent 2026 updates, including increases to FHG premiums and overall fee adjustments, optimizing billing has become even more important.

In many cases, improving how you bill can have a meaningful impact on income without increasing workload.

Opening a medical clinic in Ontario is not just about construction — it’s a structured process with multiple stages.

From lease negotiation to design, permits, buildout, and final setup, each step plays a role in how quickly (and smoothly) a clinic can open.

One of the biggest misconceptions we see is around timelines.

Even for a standard clinic, construction alone should be viewed as a minimum of 3 months after permits are in place — and that’s just one part of the process.

Planning properly from the start can make a significant difference in avoiding delays and keeping the project on track.

We broke down the full timeline here:
