Kalyna Country Primary Care Network

Kalyna Country Primary Care Network Welcome to the Kalyna Country PCN! Our doctors and staff work in the communities of Killam, Hardisty, Tofield, Vegreville, Vermilion, and Viking.

As physicians and healthcare providers working in your community, the Kalyna Country PCN is committed to providing you with programs and services designed to improve your quality of care. Primary Care Networks, or PCNs, are an innovative way of delivering exceptional health care. The networks have at their hub your family physician. It has been well documented that having your own family physician

improves your health. Through the PCN, your family doctor is connected to a wide variety of health care professionals who will ensure that you get the support, information and additional care that you need to be the healthiest you possible. 🌍 Happy Earth Day! 🌍

Today, let's celebrate the planet we call home! Here are some tips to promote your personal health while also caring for the health of our Earth.

🚲 Reduce your carbon footprint and stay active by biking to work or school. It's a win-win for you and the planet.

🌱 Embrace locally grown, in-season foods. By choosing local, you're supporting nearby farmers and reducing the environmental impact of long-distance food transportation.

🌳 Reconnect with nature! Whether it's hiking, gardening or simply enjoying a picnic in the park, spending time outdoors is good for both your mind and body.
From healthy eating to staying active, if you’re looking to improve your health and well-being, there are a variety of free workshops available through Alberta PCNs. Visit ➡️ albertafindaprovider.ca/workshops/home for more information.

Connecting Patients to Care, Resources, and Support

Hi, my name is Lisa Rochford. I’m a Community Support Navigator(CSN) with Kalyna Country Primary Care Network.

📍 Vermilion Office
🗓 Tuesdays & Thursdays | 8:30–4:30
✅ No referral needed – self‑refer

My role enables me to walk alongside patients as they navigate the healthcare system, helping with access to resources, understand their care, and feel more supported in what can often feel overwhelming.

My work focuses on addressing both practical barriers and social determinants of health, including financial strain, limited supports, system navigation challenges, and access to services.

Disconnection doesn’t always look dramatic.

Sometimes it looks like:
• Sitting in a room full of people and feeling invisible
• Not knowing who to call
• Keeping things to yourself because it feels safer

We chose this year’s theme: Come Together, Canada because connection protects mental health.

When we feel seen, heard, and valued, we are more resilient.
When we belong, stigma loses its grip.
When we connect, we create pathways to care.

Caregiving can take many forms. It can look like helping a parent, supporting a partner, or caring for a child or family member. It’s often done quietly and over time, it can take a toll.
That’s why it’s important to check in with yourself and recognize the signs of burnout. Caregiver burnout is a state of physical, emotional and mental exhaustion resulting from prolonged, chronic stress. Key symptoms include:

➡️Constant fatigue
➡️ Sleep disruptions
➡️ Irritability and hopelessness
You don’t have to carry it all on your own. Your PCN is here to support you and the people you care for.
From mental health support to connecting you with local programs and resources, your PCN can help you stay on top of your own health while you care for others. Visit https://kalynapcn.ca/community-support-navigator/ to learn more.

Your wellness journey starts with something as simple as a conversation.

A Prescription to Get Active (RxTGA) begins by asking your healthcare provider for a prescription.

Your healthcare professional will review your health history, discuss your goals and determine if activity is appropriate. They can provide a written prescription that connects you with participating facilities for a variety of activities, online programs and more opportunities for activity in your community.

Clear guidance, and a practical FREE first step. If you would like support getting started, bring it up at your next visit.

Today, Tuesday, April 7 is World Health Day 🌍🩺

This year's theme, “Together for health. Stand with science”, celebrates the power of scientific collaboration to protect the health of people, animals, plants and the planet.

Research shows patients who have a continuous relationship with a family doctor and team are healthier, live longer and are hospitalized less.

KCPCN's collaborative, team-based approach to primary care can help you achieve and maintain your health goals.
