South Georgian Bay Community Health Centre SGB CHC provides community focused health promotion, illness prevention & client centred primary hea

The (SGB CHC) services the residents of Blue Mountains, Clearview Township, Wasaga Beach, Collingwood and Elmvale. A range of community-based health services are provided by a multi-disciplinary team including physicians, nurse practitioners, nurses, a social worker, dietitian, physiotherapist, health promoter, diabetes educator, mental health and addictions counsellor, a community youth services coordinator, administrative staff, our dedicated volunteers and you.

You do not need to be a client to take part. Our programs are welcoming, inclusive and accessible, helping people of all ages and backgrounds feel supported, connected and empowered.
