As we reflect on 2025, we’re grateful for the opportunity to work alongside our partners and community on initiatives that support local health and well-being - including projects like the Aging Well resource ( https://buff.ly/NSvAGyZ ), the Tillsonburg Unattached Care Clinic ( https://buff.ly/XTDvQM9 ), and continued collaboration on the Oxford Homelessness and Addiction Recovery Treatment (HART) Hub initiative ( https://buff.ly/AyRl03y ).

As we look ahead, we remain committed to listening, learning, and responding to community needs in 2026. Happy New Year!

Join us as we say thank you and farewell to our student team member, Matt.

Matt worked with our team during a semester in 2024 and returned again this past fall. Throughout his time with the Oxford OHT, he brought thoughtful insight as an Applied Health Information Science student - supporting key projects through the development of evaluation tools, metric plans, and helping us clearly define what success looks like. His contributions were especially impactful for initiatives such as the Tillsonburg Unattached Care Clinic.

Reflecting on his time with us, Matt shared:
“My time working with the Oxford Ontario Health Team was a genuinely meaningful and rewarding experience. Being from Tillsonburg myself, it was especially meaningful to contribute to projects that directly support the people and places I call home. I learned firsthand how collaboration, data, and thoughtful planning come together to improve care. The trust, guidance, and support I received helped me grow both professionally and personally. I’m truly grateful to have been part of work that makes a real difference for patients.”

We’re grateful for Matt’s contributions and wish him all the best as he continues his academic and professional journey. Thank you, Matt!
