Earlier this month we got together in-person for our monthly Leadership & Strategy Council meeting.

At the meeting, we heard powerful examples of the Oxford Primary Care Network in action:

➡️ 753 residents from the pre-January 1, 2025 Health Care Connect waitlist are now attached to ongoing primary care through the Ingersoll Nurse Practitioner Led Clinic 👏

➡️ The Tillsonburg Unattached Care Clinic (TUCC), opened in July 2025 and operated by the Ingersoll NPLC, has already provided care to more than 1,300 people without a primary care provider ✅

➡️ When a complex patient needed support to leave hospital, primary care clinicians across Oxford stepped up. After outreach from OHT Primary Care Physician Lead Dr. Tamachi, a Tillsonburg family physician agreed to assume ongoing care - helping the patient transition home and freeing a hospital bed for another person 🏥

We also:

➡️ Heard from our Palliative Care Clinical Coach, Diane, about her coaching impact locally for 2025/26 and expansion into 2026/27 and

➡️ Enjoyed a thoughtful treat from Community Council Members Maureen and Deb as a thank you for the L&SC's hard work and dedication over the last year 🎂🎉

Day 2!

Yesterday evening, Emily and Malaysia attended the Integrated Foundation for Integrated Care Canada Reception. Bringing together colleagues, partners and friends on the evening of the first day of the conference, this reception offered an opportunity to reconnect, meet fellow delegates and share in the work we are doing together to advance integrated, people-centred care.

Today, Emily will be attending the "Collaboration for Transformative Action in Integrated Primary Health Care" session. This workshop will show how working together can improve primary health care and make it more fair for everyone. The session will share a new partnership between national organizations, hear ideas and experiences from participants, and explore how working in alignment and using each other’s strengths can lead to real, positive change.

Join us in congratulating Emily and Malaysia on the presentation of their oral paper, “Co-designing recovery together – Addressing homelessness, addictions, and social inequity in rural Ontario, Canada,” at in Birmingham, UK.

This presentation highlighted the Oxford HART Hub, a project built with and for people who face barriers to care, including people experiencing addiction, mental health crises and homelessness. It focused on the community-led sessions held across Oxford County, how people with lived and living experience helped shape what care should look like, and the benefits, implications, and outcomes of this model of co-design.

Presented as part of a session on co-designing care with people experiencing inequity, this work shows how listening, building trust, and working alongside community can lead to more compassionate and effective care.

We’re so proud to see this important work shared on an international stage. Way to go, Emily and Malaysia!

OHT Community Engagement Project Coordinator, Emily, and Southwestern Public Health Health Promoter and former OHT student, Malaysia, have arrived at the International Conference on Integrated Care (ICIC26) in Birmingham, UK!

The overall conference theme is "Integrated Care for all: Promoting Health and Wellbeing Through Diversity." Throughout the next few days, the team will attend themed sessions, learn from other providers from across the globe, and make meaningful connections with a diverse group of health professionals.

This afternoon they will present their Oral Paper: "Co-designing recovery together – Addressing homelessness, addictions, and social inequity in rural Ontario, Canada." 👏 Stay tuned!

We're thrilled to have OHT Community Engagement Project Coordinator, Emily, representing us at the International Conference on Integrated Care (ICIC26) in Birmingham, UK next week!

Emily will be joined by Southwestern Public Health Health Promoter and former OHT student, Malaysia, to present their Oral Paper titled "Co-designing recovery together – Addressing homelessness, addictions, and social inequity in rural Ontario, Canada." This paper is one of four being presented in a session about "Co-Designing Care with People Experiencing Inequity" and will focus on the development of the Oxford HART Hub.
