The Oxford OHT is thrilled to be at the 2025 Integrated Care Action Summit!

This two day event is an opportunity to connect with other Ontario Health Teams, primary care partners, patient and caregiver advisors from across the province who are all working toward the same goal: making care more connected and easier to navigate.

Over the next two days, we’ll be learning from others, sharing what’s working in Oxford County, and exploring new ideas to strengthen collaboration across the system.

We’re looking forward to the conversations, the energy, and the chance to keep pushing integrated care forward together!

These vaccines are available for free for all individuals 6 months of age and older who live, work or go to school in Ontario.

Where to get the shot:

This Digital Health Week, we're celebrating the impact of AI Scribe tools across Oxford County.

With support from the Oxford Ontario Health Team, local clinicians are using AI scribing tools to reduce administrative burden and improve the patient experience - and the early results are promising!

📌 Over 85% of clinicians who use AI Scribe are satisfied or very satisfied with the tool.

📉 82% say it meaningfully reduces their administrative workload, helping them spend less time on paperwork.

📝 Nearly 90% report clearer, more complete documentation, improving the quality of clinical notes.

🤝 60% say they can now spend more time in direct patient care, supporting more relationship-focused visits.

⏳ Clinicians who have used the tool for 6+ months report even higher satisfaction, showing that value grows with routine use.

Last week our Project Coordinator, Emily attended the "OHT In Good Relation Gathering" hosted by the Indigenous Primary Health Care Council. This gathering was an opportunity to learn about Indigenous cultural safety and two-eyed seeing, which is a teaching that directs attention to "learning to see from one eye the strengths in Indigenous knowledge and ways of knowing, and from the other eye the strengths in Western approaches."

Attendees were also introduced to the new "ACTION framework" that is still in development and will help our OHT in our work towards reconcili-ACTION through a circle of transformation.

Meet Jessica Carrasco-Harding, Nurse Practitioner at the Tillsonburg Unattached Care Clinic and Ingersoll Nurse Practitioner Led Clinic.

Jessica provides Primary Care for individuals who don’t have a regular Primary Care provider - often people who have gone long periods without access to consistent health support. “When they walk through our doors, they may be feeling frustrated, worried, or simply worn down from trying to navigate the system on their own. My role as a Nurse Practitioner is to change that experience - to listen, to validate their concerns, and to give them a clear path forward.”

In this role, Jessica meets with a variety of people, from concerned parents of a newborn to an older adult managing a chronic condition, to someone reaching out for mental health support. In every situation, Jessica emphasizes the importance of making sure that person leaves feeling seen, heard, and with a sense of control over their health.

“The joy that comes from watching someone finally feel relief, validation, or hope, that’s what keeps me passionate about this work.”

Last week, we were proud to bring together navigators, healthcare leaders, and community organizations from Elgin, Oxford, and Middlesex-London to continue advancing health system navigation and equity across our region.

Key highlights included:

📞 Health811 updates: expanding digital and live-chat navigation supports, new partnerships with 988 for mental health crisis response, and upcoming initiatives to better assist caregivers and non-clinical inquiries.

👩‍⚕️ HealthCare Connect insights: updates on provincial strategies to connect people to primary care physicians and nurse practitioners and a look at how 235,000 Ontarians are being supported on the waitlist for primary care.

🤝 Cross-OHT collaborations: participants shared projects that embed community navigators and intake coordinators in local health centres to ease the burden on physicians and help connect residents directly to care.

🌍 Health Equity Panel: powerful conversations on Two-Eyed Seeing in Indigenous services, newcomer experiences navigating language and cultural barriers, and the role of mobile and community-based services in reaching people where they are.

We’re grateful to all the speakers, partners, and participants who joined us to share their insights and strengthen our collective work toward a more connected, equitable, and person-centred healthcare system.
