A new trial just published looked at whether people with chronic non-specific low back pain did better with Pilates when told to keep the abdomen contracted or relaxed.
The result?
Keeping the abdomen relaxed led to a slightly better improvement in disability, while pain, function, and global improvement were basically unclear or similar between groups.
So once again, the sacred commandment of
ābrace your coreā takes another punch to the throat.
To be clear, this doesnāt prove abdominal bracing is evil.
It just shows that in this setting all the obsessive cueing about deep stabilisers and core activation doesnāt add anything particularly special.
That fits with what many of us have been saying for years.
A lot of rehab helps not because of perfect muscle sequencing, magical motor control, or some 90s stability fantasyā¦
But because people move more, gain confidence, and stop treating their spine like IKEA furniture.
Pilates can help low back pain.
But the idea that it only works properly if people are constantly clenching their abs like theyāre being punched in the stomach is daft.
