What Will Happen If Light (Lasers) Is Absorbed By Skin Tissue?
1. Photo Stimulation
Light stimulates the plant nerves (sympathetic and parasympathetic nerves), which changes the plant nerves, resulting in changes in body function and bringing cosmetic effects. Also, there is evidence that low energy density lasers accelerate wound healing.
2. PDT (Photo Dynamic)
A method by which light hits tissue and causes a chemical reaction or series of chemical reactions within the tissue leading to effective treatment or improvement of the affected area (target tissue). (For example: Photodynamic treatment of acne)
- Propionibacterium acnes (PAC) produces endogenous porphyrins in normal metabolic process. (A)
- Specific wavelength of light can pe*****te the skin and be absorbed by porphyrins to stimulate the porphyrins to release singlet oxygen ions. (B)
- Singlet oxygen ions can effectively kill propionibacterium acne. (C)
3. Photo-Thermal Effect
- Thermal Effect
Temperature: 60~65℃, the collagen tissue changed from triple helix structure to arbitrary curling structure, the length becomes 1/3 of the original, and the diameter increases. The triple helix structure and arbitrary curling structure can be converted to each other freely, so the thermal effect is not lasting.
Representative tech: RF, ThermoLift
Clinical effect: Tightening and lifting. It does not affect tissue activity, no heat necrosis, no inflammation, no redness, no swelling, no burning, no pain, no complicated post-treatment care.
- Thermal Coagulation
Temperature: above 75℃, there must be thermal effect surrounding thermal coagulation. Thermal coagulation is related to temperature, time, and the amount of light acting on the tissue. The bigger the pulse width, the more heat coagulation, and more inflammation, more redness, swelling, heat and pain, which causes irreversible inactivation on the tissue, so the post-treatment care is much more important,
(1) Inflammation control -- Cactus Polysaccharide
(2) Moisturizing -- EGF skin growth factor, Hyaluronic acid
(3) Sunscreen -- Sun cream (Titanium dioxide, Zinc oxide)
(4) Rapidly promote skin repairing -- EGF skin growth factor, Hyaluronic acid, etc.
(5) Whitening, spot clearing, pigmentation reducing -- Tranexamic acid, Polyols (apply locally in inflammatory period) and kojic acid derivatives. Azone, Enzyme (apply locally on spots areas after inflammatory period).
Representative tech: E-light, Ulthera, lasers like 980nm laser, 808nm diode laser, 1064nm, 755nm, Liposonix.
Clinical effect: Proliferation, remodeling while lifting, tightening and whitening.
- Thermal Ablation
Temperature: above 300℃, and enough time. There must be thermal coagulation surrounding the thermal ablation spot, and thermal effect surrounding thermal coagulation.
There are 4 thermal ablative lasers: 1550nm, 1970nm, 2940nm, 10600nm. Their target tissue is water.
Thermal ablation depth is related to laser wavelength, scanning frequency, water content of the tissue and energy density. The amount of thermal coagulation surrounding the thermal ablative hole is related to the pulse width (time, T). All the fractional lasers realize energy adjusting through pulse width (time, T) adjusting, so we need to be very careful to choose the pulse with.
Thermal ablation is rarely used in the treatment of large dermal lesions but it can be used in large epidermal lesions such as coffee spots. To improve skin aging and photo aging (wrinkle and skin texture improvement), we must apply large and scattered thermal ablation (minimally invasive spots) through the dermis to the tissue. If the treatment is on faces, please shot the whole face to avoid pigmented uneven skin tone after treatment.
Representative tech: CO2 laser 10600nm, Er:YAG 2940nm, Fractional 1550nm, 1970nm.
Clinical effect: Laser micropore introduction (wall breaking effect); Proliferation and remodeling; Firming and lifting.
If we want to get good clinical results, pulse width and focal spot density need to be carefully and appropriately selected, to build thermal "bridge" among minimally invasive spots.
