Chronic kidney disease (CKD) is progressive loss in kidney function over a period of months or years. The symptoms of worsening kidney function are not specific, and might include feeling generally unwell and experiencing a reduced appetite. Often, chronic kidney disease is diagnosed as a result of screening of people known to be at risk of kidney problems, such as those with high blood pressure or diabetes and those with a bloodline relative with CKD. This disease may also be identified when it leads to one of its recognized complications, such as cardiovascular disease, anemia, pericarditis or renal osteodystrophy (the latter included in the novel term CKD-MBD). CKD is a long-term form of kidney disease; thus, it is differentiated from acute kidney disease (acute kidney injury) in that the reduction in kidney function must be present for over 3 months. CKD is an internationally recognized public health problem affecting 5–10% of the world population.

Causes of diabetic nephropathy
Kidneys are consisted of millions of nephrons. And these nephrons can work together to filter metabolic products, toxins and extra fluid, etc out of the blood. However, when exposing to high blood sugar level for a long time, these tiny blood vessels will be damaged gradually. As a consequence, diabetic nephropathy occurs.
In addition to high blood sugar level, other factors including high blood pressure, hyperlipidemia, genetic factors are also associated with the onset of DN.
Stage of diabetic nephropathy
In clinical, diabetic nephropathy is divided into five categories:
Stage 1: This stage is also known as high filtration stage and the GFR value is usually above 150. According to the statistics, the size of kidney will increase by 20% and the endogenous creatinine clearance rate increases by 40%.
Stage 2: After about 18-24 months course of diabetes, the glomerular basement membrane will become thickened. And GFR level ranges from 130-150.

Stage 5: Also known as ESRD, the GFR level is lower than 20 and creatinine level is above 400. To expand the life span, dialysis or kidney transplant are commonly used.

Diabetic nephropathy is a clinical syndrome characterized by the following :
Persistent albuminuria (>300 mg/d or >200 μg/min) that is confirmed on at least 2 occasions 3-6 months apart
Progressive decline in the glomerular filtration rate (GFR)
Elevated arterial blood pressure (see Workup)
proteinuria was first recognized in diabetes mellitus in the late 18th century. In the 1930s, Kimmelstiel and Wilson described the classic lesions of nodular glomerulosclerosis in diabetes associated with proteinuria and hypertension.
By the 1950s, kidney disease was clearly recognized as a common complication of diabetes, with as many as 50% of patients with diabetes of more than 20 years having this complication.
Currently, diabetic nephropathy is the leading cause of chronic kidney disease in the United States and other Western societies. It is also one of the most significant long-term complications in terms of morbidity and mortality for individual patients with diabetes. Diabetes is responsible for 30-40% of all end-stage renal disease (ESRD) cases in the United States.
Generally, diabetic nephropathy is considered after a routine urinalysis and screening for microalbuminuria in the setting of diabetes. Patients may have physical findings associated with long-standing diabetes mellitus. Good evidence suggests that early treatment delays or prevents the onset of diabetic nephropathy or diabetic kidney disease.
Regular outpatient follow-up is key in managing diabetic nephropathy successfully.

Can kidney disease patients eat fish?
Occasional consumption of fish is okay. Nephrotic patients can eat freshwater fish, the unsaturated fatty acid content is high. Freshwater fishes are suitable for patients with blood lipids, High blood pressure.
Carps are most suitable for nephrotic patients to eat. Occasional consumption of carp can relieve and stabilize the condition. But some of fish have a stimulating effects on the disease condition, such as silver carp, which should be avoided by the patients.

Can kidney disease patients eat fish?
Deep-sea fish are not recommended for consumption.
