FES vs AFO — Understanding the Difference

Functional Electrical Stimulation (FES) focuses on active rehabilitation by stimulating muscles and supporting neural re-education.
It helps users train muscles actively and achieve a more natural gait pattern.

Ankle-Foot Orthoses (AFOs), on the other hand, provide passive support.
Long-term use may limit neural recovery, contribute to muscle disuse, and result in a stiffer walking pattern.

Choosing the right solution depends on rehabilitation goals —
support alone, or active recovery.

📌 AFO and FES: Different Roles in Rehabilitation
Research has shown that prolonged lack of active muscle use is associated with disuse muscle atrophy.
When muscles are not actively engaged, muscle strength and mass may gradually decline.

Ankle-Foot Orthoses (AFOs) provide important mechanical support for gait, but they mainly serve a compensatory role, offering external assistance rather than activating muscles or neural pathways.

In contrast, Functional Electrical Stimulation (FES) actively stimulates muscle contraction.
Studies suggest that FES can support muscle strengthening, help counteract disuse atrophy, and promote neuromuscular activation, with potential benefits for neural plasticity and functional recovery.

👉 These findings highlight the importance of active muscle engagement in rehabilitation, beyond mechanical support alone.
