Never do this: placing a one-piece IOL in the sulcus. While it may seem like a quick fix, the long-term risks are simply too high.
This can lead to pigment dispersion and, in severe cases, UGH Syndrome (Uveitis-Glaucoma-Hyphema). Over time, these can result in permanent glaucoma or chronic uveitis.
If a capsular complication occurs, it is always safer to leave the eye aphakic and plan for a secondary procedure. The best solution, when possible, is always a three-piece IOL.
Patient safety must be our top priority!
