Untreated cavities (dental caries) can progress deeper into the tooth. At first, decay affects only the enamel and dentin, but if left untreated, bacteria can reach the pulp (the inner tissue with nerves and blood vessels). Once the pulp becomes inflamed or infected, pain, swelling, or abscesses can occur — and at that stage, a root canal treatment (endodontic therapy) is often needed to save the tooth.
