So - who are we?
We will start by introducing Pavel Nevicky who is Mindpax' CEO and living in Prague, Czech Republic.

Dear Mindpax Community 💙👋🏻

Tonight we would like to present a project by the Canadian filmmaker Kalina Bertin. In collaboration with her bipolar siblings, psychiatrists and scientists at the Douglas Mental Health Institute, as well as the MIT McGovern Institute for Brain Research in Cambridge and the Stanley Center for Psychiatric Research at the Broad Institute, she has created a virtual reality world that makes it possible to experience the world of manic depression. The project was presented in 2019 at the Ernst Schering Foundation in Berlin.
It aims to raise awareness and build empathy for the real conditions of bipolar disorder and to promote destigmatization. In addition, the VR world can help to create a better way of dealing with the patient, also for hospital staff and relatives, since the VR experience can promote a more unprejudiced approach to the disease by all those involved.

Within VR, people see and experience poetic hallucinations, altered sensory perceptions and beautiful but also frightening impressions that can accompany manic depression. On Vimeo, the term "Manic VR Walkthrough" can be used to give an impression. 🥽👀👩🏻💻

Dear Mindpax Community! 💙👋🏻

A 2016 study has investigated the association and effect of thyroid hormones on bipolar disorder. For the study, the researchers investigated brain sugar metabolism using positron emission tomography and [F-18] fluorodeoxyglucose before and 6 weeks after treatment with L-T4 or a placebo. The administration of thyroid hormone reduced the rate of depression and reduced brain activity in the limbic system. However, the exact mechanisms underlying this clinical improvement are still unknown.

The researchers say that the results clearly show that the administration of supraphysiological amounts of thyroid hormone improves symptoms in patients with bipolar disorder. This appears to occur by modulating brain functions in the parts of the anterior limbic system. Furthermore, the results confirm the long known close connection between thyroid gland and depression.

Watch out! Today we present another exciting bipolar fact! 🤓

Bipolar disorder is associated with extreme mood swings. Researchers have investigated what the phases of the disease cause in the brain over the long term. Using MRI brain scans, 21 patients were examined within 4 years. During this time, each of the participants experienced at least one episode of the disease. Some of the test persons even went through up to 6 phases. The evaluation showed that especially the grey mass was lost in areas responsible for memory and coordination. In the brains of mentally healthy control subjects, the researchers could not detect any such loss. The more often a patient slipped into a manic-depressive phase during the study period, the more brain substance was lost. The study directors established a correlation here with the number of episodes.

However, it must be emphasized that every person loses grey matter over time. In addition, although the patients performed worse in cognitive performance tests in the following years, the losses were very small and not significantly worse than before.

In further studies, the scientists want to find out why the brain substance degrades and whether stress hormones possibly contribute to this.

Today we will introduce a technical term to you. "Cyclothymia" describes the condition of mild hypomanic and minimally depressive phases with irregular progression and lower severity than in the bipolar disorder. According to the diagnostic criteria, these symptomatic periods must occur for more than half of the day for a period of more than 2 years. The diagnosis is made clinically on the basis of appropriate medical history sheets. The treatment of cyclothymia consists mainly of psychoeducation.

Cyclothymia often precedes a bipolar 2 disorder. However, it can also manifest itself as extreme mood lability without developing into a severe affective disorder. In some people a cyclothymic and chronically hypomanic disposition can contribute to success in work, performance and artistic creativity, but it also has negative effects on social relationships and impulsive behavior.

Today we would like to tell you something about the magic effect of the hormone "oxytocin". Tactile sensation is at the center of our experiences in the world. It shapes the way we perceive our environment, especially in close relationships with other people. The sense of touch is one of the central forms of perception, although it is often subordinated to visual processing.

Social touch is a prerequisite for intimacy and is of great importance for the formation of trust-based relationships. The establishment and maintenance of social hierarchies are modulated by various neurotransmitters in the brain.

A recent study reports that, for the first time, researchers have succeeded in taking electrophysiological images of individual oxytoxin neurons in freely moving female rats. Thus, it could be proven that oxytocin neurons are activated specifically by physical contact. The study showed that a small group of oxytocin neurons, so-called parvocellular oxytocin neurons, are responsible for the conversion of sensory signals into social interactions. Thus, the communication between the female rats was enhanced.

These findings could lead to the development of new therapeutic approaches using oxytocin nasal sprays to help patients affected by posttraumatic stress disorder, depression, autism or social anxiety disorders, for example.

Today we would like to inform you about the economic costs of bipolar disorder. As you can probably already imagine, these are very high and it is immensely important that we talk about them so that more space can be given to public education, destigmatization and the demand for further training of the medical profession.

A survey from 2004 discovered that the annual costs arising from bipolar disorders in Germany sum up to approximately €5.8 billion. For the most part, this is due to the so-called indirect costs: these include all costs that can be traced back to sickness-related unemployment, suicide-related production losses, and work and occupational disability. Furthermore, 2/3 of the direct costs are caused by the very high average hospitalization of patients for inpatient treatment.

The diagnosis of bipolar disorder is mainly based on the ICD-10 classification. This leads to a conservative estimate of the medical costs, as this is a relatively narrow definition of the disease. However, the long period of time between the first acute episode & the correct diagnosis also leads to a delayed phase prophylaxis and thus to increased medical costs.

It can often take years before a bipolar disorder is correctly diagnosed. Approximately 35% of patients have symptoms for 10 years before they are correctly diagnosed and can start a therapy.

The first symptoms are often confused with depression, as a bipolar disorder can begin with a depressive episode. However, a premature therapy with antidepressants can have a negative effect on the course of the disease and trigger, for example, manic symptoms or rapid cycling.

In contrast, especially in hypomanic phases, patients are often perceived as particularly creative, self-confident and capable.

Complications in the early detection of the bipolar spectrum of disease can have devastating consequences for those affected, which is why those providing treatment should also include relatives in the anamnesis. This can help to obtain a comprehensive picture of the existing symptoms and facilitate rapid treatment.

It is often claimed that people with bipolar disorder are particularly creative. The flood of ideas from mania or hypomania can actually bring a lot of creative ideas and imaginations. Thus, the environment of those affected can also experience them as particularly powerful and agile. Studies also show a connection between creativity and bipolar disorder. Scientists estimate that bipolar disorder is 10 to 40 times more common among artists than in the general population.⠀

Creative activity during manic episodes can not only help to channel excess energy and ideas, but can also help in retrospect to better understand the sensation and the disease.⠀

Famous bipolar artists include Vincent van Gogh, Kanye West, Virginia Woolf, Hermann Hesse, Napoleon Bonaparte and Ludwig van Beethoven, to only name a few.⠀
