Adipositas tritt oft zusammen mit einer Reihe von Stoffwechselerkrankungen auf, die das so genannte metabolische Syndrom bilden. Dazu gehören schwere Stoffwechselstörungen wie Insulinresistenz, Dyslipidämie, Diabetes mellitus Typ 2 und andere. Diese Krankheiten gelten auch als kardiovaskuläre Risikofaktoren: Sie scheinen die Entwicklung von arteriosklerotischen, kardiovaskulären Krankheiten unmittelbar zu begünstigen. Herzinfarkt und Hirninfarkt sind dann die ungünstigsten Endpunkte. Darüber hinaus führt Dyslipidämie zur Steatohepatitis (nicht-alkoholbedingte Fettleber), einer chronischen Lebererkrankung, die sich dramatisch in eine Leberzirrhose umwandeln kann. Die metabolische Chirurgie bietet hiervon betroffenen Patienten ausgezeichnete Chancen, indem sie eine vollständige Heilung des Diabetes mellitus Typ 2, der Dyslipidämie und der Insulinresistenz herbeiführen kann. Auch der Zustand bei Steatohepatitis kann verbessert, die Zirrhose abgewendet werden. Bezüglich des Diabetes mellitus Typ 2 wird die Insulintherapie nach der Operation oft überflüssig, unabhängig von der Menge des zuvor benötigten Insulins. Eine Reduktion von 300 IE Insulin auf Null ist möglich. Die Verbesserung der Dyslipidämie tritt normalerweise innerhalb eines Jahres nach der Operation, parallel mit der Gewichtsreduktion, ein. During the landmark 30th Conference of iSMIT & IBEC in Seoul (2018), global leaders in surgery, biomedical engineering, and digital innovation united to shape a common vision for the future of healthcare.
This vision culminated in what is now known as the Seoul Manifesto:
“The Seoul Declaration: A Manifesto for Ethical Medical Technology”
Co-authored by Prof. Young-Woo Kim President of iSMIT2018 and Prof. Paul Barach, the declaration was formally signed by the conference participants.
The manifesto highlighted:
Stronger collaboration among engineers, clinicians, and policymakers
