The "Cystic" Detective (LAM vs. LCH vs. BHD)

The Weirdness: You see a patient with a "swiss cheese" lung full of cysts. They all look the same to the untrained eye, but the "smart" pulmonologist knows the shape tells the story.

• The "Smoker's" Cyst (Langerhans Cell Histiocytosis - LCH):
• The Shape: Bizarre, irregular shapes. Think "Clover-leaf" or "bizarrely thick-walled."
• The Zone: Strictly Upper/Mid zone. The costophrenic angles are spared (LCH hates the bases).
• The Pearl: If the cysts look "ugly" and the patient smokes, it's LCH. Stop smoking, and it often stabilizes.

• The "Estrogen" Cyst (Lymphangioleiomyomatosis - LAM):

• The Shape: Perfectly Round, Regular circles. Thin-walled.
• The Zone: Diffuse. Everywhere. No sparing of the bases.
• The Pearl: Exclusive to women (usually). Look for Angiomyolipomas in the kidney or chylous effusions.
• The Check: If you suspect LAM, check VEGF-D levels. If >800, you don't need a biopsy.

• The "Renal" Cyst (Birt-Hogg-Dubé - BHD):

• The Shape: Lentiform (Oval/Lens-shaped).
• The Zone: They love the Basal and Medial lung zones, often hugging the mediastinum or fissures.

• The Killer: These patients get Renal Cell Carcinoma. The lung cysts are the "alarm" to save their kidneys. Look at the face for tiny skin bumps (fibrofolliculomas).
