AJOG Expert Review in Labor: A critical evaluation of the external and internal maneuvers for resolution of shoulder dystocia - The anatomy of the pelvic outlet and fetal shoulders
Anteriorly and anterolaterally, the pelvis is bound by the bony p***c arch and ischial tuberosities on both sides. Posteriorly, it is bound by the tip of the sacrum. In shoulder dystocia, the fetal shoulders are stuck at the bony anteroposterior dimension of the pelvis. However, postero-laterally, the sacrotuberous ligaments and other soft tissues allow some expansion and make the diagonal dimension of the pelvic outlet more spacious for the insertion of the accoucheur’s hand and the delivery of the fetal shoulder. https://ow.ly/Lwvx50PB52g
