Tip of the Week Tuesday!

Composite flexion tightness is present if the difference between active wrist flexion and composite digit and wrist flexion is greater than 10 degrees. Effectively treat this condition by isolating the wrist muscles and re-establishing tenodesis. (See videos in comments.)

1. Gently wrap their fist in Coban, not as a stretch, but simply to hold the digits at the end range. Ask them to perform 2 minutes of gentle active wrist flexion and extension with the proximal joints stabilized on a wrist wedge.

2. While keeping the digits wrapped, have them perform passive wrist flexion for 2 minutes with a series of gentle 20-second holds.

The forearm is made up of group of muscles located both at the anterior and posterior part.

These muscles are unique in terms or function and to identify the individual muscles which make up up the anterior part of the forearm, remains an important area to know off head as health care worker, especially a physiotherapist.

The group of muscles that make up the posterior part of the forearm have their tendons joined together and insert at the lateral part of the elbow (lateral epicondyle of the elbow). When there is irritation of this part, a lateral epicondylitis is caused.

When these muscles at the posterior compartment of the forearm contract, the wrist is extended.

However, those found at the anterior compartment, are entirely different and function differently too. These muscles are also having their tendons joined and inserted at the medial side of the elbow(Medial Epicondyle). It is a point which gets irritated all the time when there is medial epicondylitis.

When these muscles contract, the wrist is flexed. Following stroke, these muscles tend to shorten to cause the wrist in a fixed flexion position. When the wrist is flexed, there is a direct link for fingers flexion too. This is why in most of the cases in stroke, such result to a claw-like hand.

These muscles include the following;
1. Flexor Carpi Ulnaris

2. Palmaris Longus

3. Flexor Carpi Radialis

4. Pronator teres.

The interface between the adductor pollicis muscle of the thumb and the first dorsal interosseous muscle is a common area of adhesion. When adhered, this can lead not only to tightness of the thumb, but also poor grip and DeQuervain's tenosynovitis. A helpful stretch for it goes like this. Make an "L" shape between your thumb and index finger. Hook your thumb onto something, such as a table, to hold it in place as you use your other hand to pull the index finger back behind your middle finger. You should feel a great stretch.
