📙Check list of main pulmonary function test
1)Spirometer ▶️▶️▶️measure by FVC , FEV1 , FEV1/FVC
2)Lung volume ▶️▶️▶️measure by by FRC , RV , TLC
3)Diffusion ▶️▶️▶️measure by DLCO (Single breath test)
✅اولا لازم تعرف هي Obstructive ولا Restrictive
هعرفها من FEV1/FVC ◀️◀️◀️لو اكتر من 70% يبقي Restrictive
◀️◀️◀️لو اقل من 70% يبقي Obstructive

✅طيب لو Restrictive ◀️◀️◀️ يبقي FVC أقل من 80%
وتتاكد من TLC ◀️◀️◀️ يبقى اقل من 80%

✅طيب ازاي تعرف سببها
تبص علي DLCO ◀️◀️◀️لو اكثر من 80% يبقي Chest wall disease ◀️زي Kyphosclosis , morbid obesity, neuromuscular disease

📙اما في حالات Obstructive نعرف انها ممكن تكون COPD او Broncial Asthma
نعرفها بالطرق الاتيه
1)Bronchidilator reversibility test
يعني يعمل spirometer وبعدها جلسة بخار موسع للشعب
وبعدها بربع ساعة نعيد spirometer

✅لو فيه زيادة في FEV1اكثر من 10%◀️◀️◀️Asthma
✅ونبص علي TLC لو اكثر من 120% ◀️◀️◀️COPD
ولو طبيعية ◀️◀️◀️Asthma

✅وبنص علي DLCO ◀️◀️◀️لو اكثر من 80% يبقي Asthma
◀️◀️◀️لو اقل من 80% يبقي COPD

📙ملحوظه
لو كل حاجة normal ماعدا DLCO اقل من 80% يبقي Pulmonary Vascular Disease as pulmonary embolism

١) تشخيص و خطة علاجية محكمة + التزام من المريض = نجاح العلاج في اقصر مدة ممكنة

٢) تشخيص و خطة علاجية اي كلام + التزام من المريض = احتمالية نجاح العلاج

٣) تشخيص و خطة علاجية محكمة + عدم التزام من المريض = فشل الخطة العلاجية

معلومة‼️‼️

👉Transversus abdominis muscle and multifidus muscle have been CONTRACT 30ms Before movement of shoulder

AND 110ms Before movement of the leg

👉 Internal oblique muscle and Transversus abdominis muscle work together
To Increase intra abdominal pressure

👉Multifidus muscle has been found to ATROPHY in people with Low Back Pain

🟥JRF ( Joint reaction force) is always higher than body weight

✅طيب كيف تقلل JRF

👉 Weight loss
Because reduce 1N of body weight will reduce 3N of JRF

👉 Reduce abductor muscle force
👉 Using cane contralateral

لانك لما تستخدم cane دا بيقلل demand of muscle
وبيقلل JRF فبالتالي pain هيقل

🟥If lateral trunk leaning due to hip abductor weakness
دا بنسميه gluteus medius gait

🟥If lateral trunk leaning due to hip joint pain
دا بنسميه (Antalgic gait)

🟥Stability of Hip region

✅لو واقف Double stance فيحصل 👈
Stimulated contraction ipsilateral and contralateral and adductor and abductors

✅أما لو واقف single limp stance فيحصل 👈
Action abductor muscle (Gluteus medius and minimums) pull pelvis toward greater trochanter of femeur

To prevent pelvis dropping on contralateral side

‼️ لذلك لما تيجي تمسك ال Cane تمسكه على
contralateral side

☑️ ليه بقي لانك لما تستخدم Cane بالطريقة دي

👈 يزداد moment arm وبالتالي muscular force بتقول وبالتالي Joint reaction force تقل

👈ولو عملت test لل muscle ممكن patient يعمل substitution

🟥Hip flexion

✔️ Muscle 👉psoas major
👉Iliacus

💥Origin of psoas major
👉 Transverse process (L1-L5)
👉 Vertebral bodies (T12-L5)

💥Origin of Iliacus
👉Iliac fossa

💥Insertion of psoas major and Iliacus
👉Lesser trochanter of femur

💥Nerve supply
👉 Lumber plexus

✔️ Range 👉 115-125

✔️Stabilization during motion
👉Contraction abdominal muscle

✔️ Weakness of hip flexor
👉 Difficult in climbing stair , walking up and down incline
👉Marked Weakness walking is difficult

✔️ Contracture of hip flexor
👉Uni lateral 👉Hip adduction with external rotation
👉 Bilateral 👉 increase lumber lordosis

✔️Factor limit motion
👉 With knee flexed 👉 Abdominal muscle
👉 With knee extensor 👉 Hamstring muscle

✔️ When test this muscle patient maybe substitution by

👉 Sartorius 👉 External rotation and adduction of hip
👉Tensor fascia latae 👉internal rotation and abduction of hip

🟥 Hip Extensor

✔️ Muscle 👉gluteus maximum muscle
👉 Hamstring muscle

💥Origin of gluteus maximum
👉Outer rim of ilium
👉Dorsal surface of sacrum and coccyx

💥 Insertion of gluteus maximum
👉Illiotibial tract of fascia
👉gluteal tuberosity of femur

💥 Nerve supply of gluteus maximum
👉 Inferior gluteal nerve

💥 Origin of Hamstring
👉 Semitendinosus 👉 ischial tuberosity
👉 Semimembranosus 👉 ischial tuberosity
👉 Biceps femoris 👉 ischial tuberosity

💥 Insertion of Hamstring
👉 Semitendinosus👉medial tibial condyle
👉 Semimembranosus👉 medial tibial condyle
👉 Biceps femoris👉 lateral side of fibula

💥 Nerve supply of Hamstring
👉 Sciatic nerve (Tibial part)

✔️Range
👉 extension (5)
👉Hyperextension (20)

✔️ Stabilization during motion
👉 Contract quadratus lumborum and iliocostalis lumborum

✔️ Weakness of hip Extensor
👉 Bilateral marked weakness of gluteus maximum 👉 walking extremely difficult
👉 Necessitate aid of crutches
👉 Difficult in raising trunk from forward bent position

✔️ Contracture of hip Extensor
👉 Walking with hyperextension deformity

✔️ Factor limit motion
👉 Tension of iliofemoral ligament
👉 Tension of hip flexor muscle

✔️When test this muscle patient maybe substitution by
👉 Extending lumber spine
👉 Therapist must support pelvis
