A 17 years old female patient presented to the Conservative dentistry clinic with chief complaint of restoring the tooth for function and esthetics.
Upon examination, the lingual wall was very undermined and the tooth was badly destructed.
The treatment of choice was to elevate the lingual wall and to insert a direct restoration overlay with bracing effect. EverX composite which is fiber-reinforced designed to be used as dentin replacement with the presence of fibers making it perfect substructure to reinforce composite restoration in large cavities. The direct overlay was done using injection molding technique which is a process used to create complex morphologies, establish occlusal dimensions, and mimic the structure from the index with the help of injectable resin composite of high flexure strength and wear resistance.
1- Nerve block Anesthesia was administered.
2- Removal of caries from pulpal chamber.
3- A scan was performed to print a 3D cast with the proposed treatment plan.
4- Lingual wall elevation upon the application of modified tofflemire matrix.
5- Matricing to build mesial and distal walls.
6- Insertion of EverX fiber-reinforced composite in the pulpal chamber.
7- Injection moulding of cusps, cusp by cusp using a clear silicon index fabricated on the printed 3D model.
8- Finishing and polishing of restoration.
9- Checking occlusion.
