بعد فحص عينة نسيجية مؤكدة،
وكان لديها تأثر بالغدة النخامية وأصيبت بالسكري الكاذب،
وتلقت علاجًا كيماويًا سابقًا.
حضرت الآن تشتكي من تورم ونزيف باللثة،
ومعها أشعة مقطعية تُظهر تغيرات تليفية بالكبد وبؤرًا بالطحال.
⚠️ الرسالة المهمة:
في مرضى LCH، أي عرض جديد — خاصة في الفم — لا يُعتبر عرضًا عابرًا.
قد يكون مؤشرًا على امتداد المرض لأكثر من جهاز في الجسم.

🔬 Langerhans Cell Histiocytosis (LCH): The Diagnosis Begins with a Biopsy
A female patient previously diagnosed with biopsy-proven LCH, with prior pituitary involvement and diabetes insipidus, and treated with chemotherapy, presented with gingival swelling and bleeding.
CT imaging demonstrated cirrhotic liver changes and splenic focal lesions.
In LCH, new oral findings should never be underestimated.
They may signal multisystem progression with risk-organ involvement.
The diagnosis starts with a biopsy —
but careful follow-up defines the outcome.

A 36-year-old female presented with an intramasseteric lesion.
The mass was well-circumscribed and clinically unremarkable.
🩻 Imaging:
MRI revealed a well-defined intramuscular mass within the masseter muscle, with no evidence of local invasion.
Final diagnosis:
Cavernous hemangioma.

As we begin our new year activities, we continue our mission to raise awareness about the vital role of oral and maxillofacial pathology in accurate, life-saving diagnosis.
Today, at Al Sahel Educational Hospital, Dr. Sally Ibrahim Mohamed delivered an insightful lecture entitled “Clinical Decisions in Common Oral Lesions.” The session highlighted how proper clinical judgment and early diagnosis can truly change patient outcomes.
The event was held upon the kind invitation of Prof. Samira Saeed Darwish, Head of the Dental Department at Ahmed Maher, Al-Gomhoria, and Al Sahel Educational Hospitals, as part of ongoing efforts to support scientific education and sound clinical practice.

حالة لسيدة في أوائل الأربعينات تعاني من وجود ورم على سطح اللسان مع قرحة واضحة في المنتصف.
للأسف، تأخر التشخيص وتم علاجها بطريقة خاطئة على أنها نقص في بعض الفيتامينات والعناصر الغذائية، مما أدى إلى زيادة حجم الورم واستمرار القرحة لفترة طويلة أثناء تناول أقراص حمض الفوليك دون تحسّن حقيقي.
أي ورم أو قرحة لا تلتئم لا يجب تفسيرها على أنها نقص فيتامينات فقط.
الشك المبكر ينقذ الحياة.
عند الشك في أي ورم أو قرحة بالفم، يرجى التحويل الفوري إلى جراح فم ووجه وفكين لأخذ عينة وتشخيص دقيق قبل فوات الأوان.
تم تشخيص الحالة أخيرًا: سرطان الخلايا الحرشفية باللسان.
A woman in her early 40s presented with a dorsal tongue mass with a central ulcer.
Unfortunately, the lesion was misinterpreted as vitamin deficiency and treated with folic acid for a long period, allowing the tumor to progress.
Persistent oral ulcers and masses should never be treated empirically.
Early referral for biopsy saves lives.
Final diagnosis: Oral squamous cell carcinoma.

In histopathology, always be prepared for “expect the unexpected,” and never rely solely on radiographic appearance for lesion diagnosis.
The gold standard remains the biopsy.
Here’s a case of psammomatoid ossifying fibroma in an adult female, contrary to the surgeon’s initial expectation.

Verrucous Carcinoma (VC): Why Excision Matters
VC may look harmless, but diagnosis lies at the edges, not the surface.
🔍 Why complete deep excision is essential Small biopsies can miss the key feature of VC: the transition from normal epithelium to tumor with broad pushing borders. Only excision allows:
Proper assessment of adjacent normal epithelium
Exclusion of hybrid or conventional SCC
🧬 HPV? Oral VC is not HPV-related. It is mainly linked to chewing to***co, s***f, and betel quid.
⚠️ Bottom line Excision is both diagnostic and therapeutic.
Margins matter.
A small white patch on the gingiva next to a molar. Looks simple. Feels harmless.
But location and surface say: don’t ignore it.The real question isn’t the diagnosis…
It’s observe or biopsy?
