Verrucous Carcinoma (VC): Why Excision Matters
VC may look harmless, but diagnosis lies at the edges, not the surface.
🔍 Why complete deep excision is essential Small biopsies can miss the key feature of VC: the transition from normal epithelium to tumor with broad pushing borders. Only excision allows:
Proper assessment of adjacent normal epithelium
Exclusion of hybrid or conventional SCC
🧬 HPV? Oral VC is not HPV-related. It is mainly linked to chewing to***co, s***f, and betel quid.
⚠️ Bottom line Excision is both diagnostic and therapeutic.
Margins matter.
A small white patch on the gingiva next to a molar. Looks simple. Feels harmless.
But location and surface say: don’t ignore it.The real question isn’t the diagnosis…

A jaw lesion diagnosed as an osteoma completes the picture.
When the pieces come together, Gardner syndrome becomes highly suspected.

What is ameloblastoma❓️❓️ Why do margins matter❓️❓️
Ameloblastoma is a benign odontogenic tumor characterized by slow growth and locally aggressive behavior.
Surgical margins matter because
1.🔬🔬Microscopic tumor extension often exceeds clinical and radiographic boundaries.
2.Inadequate margins are a major cause of local recurrence.
Accurate margin assessment is essential for proper surgical planning and long-term control

ليه حدود الورم مهمة؟
حدود الورم هي أطراف النسيج اللي الدكتور بيشيلها مع الورم.
هي اللي بتوضح إذا كان الورم اتشال بالكامل ولا لسه في امتداد مش باين.
فحص الحدود بدقة:
يقلل فرصة رجوع الورم
يساعد الطبيب ياخد القرار العلاجي الصح
علشان كده تحليل حدود العينة خطوة أساسية في العلاج.

شاب في العشرينات.
كان عنده كيس مصاحب لسن مطمور،
اتشال بالكامل،
والعظم رجع طبيعي.
بعد سنتين،
ظهر ورم جديد في نفس المكان.
العينة المرّة دي ورم خبيث نادر
السؤال المهم:
هل الكيس اللي فات كان بريء فعلًا؟
ولا كان بداية حاجة أخطر ما كانتش باينة وقتها؟
مش كل تحسّن في الأشعة أو الشكل الإكلينيكي معناه إن القصة خلصت.
العينة والمتابعة الطويلة هما الفيصل.
A male patient in his twenties.
Previously diagnosed with a cyst associated with an impacted tooth.
The lesion was surgically removed with complete bone healing.
Two years later, a new lesion developed at the same site.
Biopsy revealed Clear Cell Odontogenic Carcinoma.
The key question:
Was the initial cyst truly benign,
or could it have represented an early, unrecognized malignant potential?
Clinical and radiographic resolution does not always equal biological certainty.
Histopathology and long-term follow-up remain essential.

خبرة الجراح بتفرق في دقة التشخيص و ده هيظهر في الحالة التالية.
أول عيّنة اتشخّصت لويكوبلاكيا،
لكن الجراح ما اطمنّش لأن شكل القرحة ما كانش مطمِّن.
بخبرته، خد عيّنة تانية من مكان أدق و اكبر و النتيجة التانية كانت ورم خبيث.
ولما النتائج اختلفت، بعت العيّنة الأولى والتانية للمراجعة، وده يوضح دقته في الإجراءات التشخيصية.
التشخيص النهائي طلع: Oral Squamous Cell Carcinoma
الحالة دي بتأكد إن
مكان العيّنة مهم،
وإن خبرة الجراح ودقته ممكن يغيّروا التشخيص كله.
The surgeon’s experience made the difference.
The first biopsy was reported as leukoplakia.
However, the clinical appearance was not reassuring, and the surgeon was not convinced.
Based on experience, a second biopsy was taken from a more representative area.
When the findings differed, both the first and second specimens were sent for review, reflecting a careful and precise diagnostic approach.
The final diagnosis was: Oral Squamous Cell Carcinoma
This case highlights that
the biopsy site matters,
and the surgeon’s experience and diagnostic precision can change the entire outcome.

The key difference between a lateral developmental cyst and a lateral inflammatory cyst lies in tooth vitality.

✔️ Vital tooth → Developmental cyst
❌ Non-vital tooth → Inflammatory cyst

🔍 This is why checking tooth vitality is always an essential step in clinical examination.

🦷 Here’s a small radiolucency on the lower right that we all meet in practice.

Radicular Cysts in a minute

Radicular cysts are the most common inflammatory jaw cysts, developing from chronic infection around a non-vital tooth root.

Cause: long-standing periapical inflammation after pulp necrosis.
Prevalence: over 50% of all jaw cysts.
Complications: bone loss, tooth displacement, secondary infection, swelling, and—rarely—malignant transformation (most often to squamous cell carcinoma).

Early diagnosis prevents complications.
