لسيدة تعانى من ضيق حرج بالشريان التاجى الرئيسى الايسر و الم شديد بالصدر مع وجود تصلب بالشرايين و صعوبه التوسيع بالطرق التقليدية
الشنيور الطبى
تقنيه حديثه تستخدم لتكسير الترسبات والكالسيوم داخل شرايين القلب والذى يمنع محاولات التوسيع بواسطه القسطره وهو من الأدوات التى تستخدم فى هذه النوعيه من الحالات لتسهيل عمليه التوسيع وتحسين نتائج القسطره و تقليل المخاطر

Monday primary team

A 65 years old male patient, diabetic and heavy smoker, presented to the ER with severe chest pain. he was in shock

ECG shows inferior STEMI with RV involvement and complete heart block.

We immediately insert a central line and start aggressive IV fluid therapy along with Vassopressor support.
Temporary pacemaker was put in place, and we shifted the patient immediately to the cath lab

CA shows totally occ. RCA with large thrombus burden.
We did a successful primary PCI with I/C aggrestat to be continued in CCU with TIMI III flow.
The rhythm and blood pressure returned to the normal immediately on the table.

يقوم حاليا قسم القلب و القسطرة بمستشفي ٦ اكتوبر الدقي بعمل حالات تغيير الصمام الأورطي عن طريق القسطرة لمرضي ضيق الصمام الأورطي بدلا من جراحة القلب المفتوح للمرضي كبار السن - وهي تقنية حديثة يتم زرع الصمام الأورطي عن طريق شرايين الفخذ بدون تدخل جراحي - علما بوجود لجنة قلب لمناظرة حالات ضيق الصمام الأورطي في السبت الاول من كل شهر
