شويه جد بقي ...انا هكلمكم عن حاجه المفروض انها بديهيات في شغل الtrauma لاي نائب ماكسيلو....بس من كتر ما انا بقيت بشوفها ...

شويه جد بقي ...
انا هكلمكم عن حاجه المفروض انها بديهيات في شغل الtrauma لاي نائب ماكسيلو....بس من كتر ما انا بقيت بشوفها مش بتتعمل حسيت ان الواحد لازم نرجع نتكلم فيها.....
١) ال maxilla ليها اربع عمدان (4 buttress )....مفيش حاجه اسمها اثبتها 2 point fixation في الاماكن اللي مش communited وهتعدي.....والله العيان بيجيلي بعد شهور من العذاب ب malunion او fibrous union وفي الحالتين مش عارف ياكل.....مش هتعرف تجيب 4 point يبقي ع الاقل حط شرايح عدله والمسامير تبقي قافشه كويس مش loose
2)التثبيت لازم يكون بين ثابت و ثابت (fixed point to fixed point)....مينفعش والمصحف اني اثبت ع عضمايه مكسوره اصلا ....لازم اوصل لاول نقطه ثابته عشان اثبت عليها المتحرك اللي مكسور اصلا....زي الحاله اللي عارضها دي فتحت lynch مخصوص عشان اعرف اوصل لل nasofrontal buttress الثابت ...و دي حاجه حضرتك بتكون بتفكر فيها قبل ما تبدأ العمليه عشان تبقي عارف هتفتح approach ايه و ال plan ايه بالظبط....فمعلش اتعب بص ع ال cuts....متعملش العمليه بناءا ع ال3d.....ال 3d دي حاجه كده تقولك الجمجمه دي متكسره او لا....العمليه تتعمل ما ال axial & Coronal cuts ....كتير حتت صغيره تبقي مكسوره ومتظهرش في ال 3D
3)دي أهم نقطه ☀️☀️☀️
-ال maxilla لازم تتسقط قبل ما تبدأ تثبتها....تعمل complete mobilization وتتاكد انها free تماما و متعتمدش ع الاوكلوجن اللي انت جايبه في الارش بار....اول ما العيان هيصحي وترجع الmuscle tone هتلاقيه مشيس وناحيه لامسه وناحيه مفتوحه ....وفي الكومنتات هسيبلكم لينك لفيديو حاله قديمه بيشرح النقطه دي🔽🔽

ده مش معناه اني مبغلطش او شغلي مفيهوش مشاكل...بس اما تبقي عامل اللي عليك و تحصل اي مضاعفات تبقي قدام ربنا ضميرك مرتاح...وتحط صباعك في عين التخين😌😅
