Oroantral Fistula (OAF)

👉 Cone beam CT (CBCT) & CT scan PNS showing a patient with OAF following extraction of Right upper second molar, with a missed root in the sinus causing dental sinusitis not responding to antibiotics.
👉 N.B. This patient had FESS, septoplasty and partial inferior turbinectomy long time ago.
👉 Management will be endoscopic removal of the missed root through a wide middle meatal antrostomy + closure of the fistula using buccal & palatal flaps.
👉 Pre-extraction radiology + early recognition of the fistula during extraction make it easy to avoid it and be prepared to close it immediately if it occurs by approximation of bony edges and closure with sutures.

Empty Nose Syndrome (ENS)

👉 This patient was suffering from nasal obstruction due to septal deviation. After the operation, he is still suffering from nasal obstruction due to total turbinectomy.
👉 Now the patient has wide nasal passages but he feels complete obstruction. This is the paradoxical nasal obstruction characteristic of Empty Nose Syndrome.
👉 Please never do this !!

A miserable story

A 4 years old female child with a small battery neglected inside the nose for more than a month. She was treated by antibiotics but, continued to have a unilateral offensive purulent nasal discharge. The battery caused destruction and perforation of the nasal septum and saddling with collumellar collapse. FB was removed. But, she will need another operation to reconstruct the septum after ttt of infection.

This is a serial CT scans of the same patient. In no.1 he was complaining of severe headache and the surgeon suggested an operation, after the 1st operation, he continued to suffer from headache. The second surgeon asked for CT no.2 and suggested turbinate reduction, although breathing was normal. After his second operation he started to develop nasal obstruction beside severe headache. The third surgeon asked for CT no.3 and suggested FESS for left maxillary and Septoplasty, which fortunately was not done. I have seen the patient last week, and surprisingly, his headache is a typical migraine !!! He has an Empty Nose Syndrome with sense of nasal blockage and dryness for no reason. With simple medical treatment, we have reached now to CT no.4 but the nose is still blocked 🤔

If you can’t help your patient, please don’t harm him 🙏

8 years old male child presented to us with left nasal obstruction and epistaxis. Endoscopic resection was done. The final view after drilling the floor of the sphenoid, vidian canal and the pterygoid wedge.

The patient had a frontal osteoma which was removed by osteoplastic flap 18 years ago. These 2 depressions in anterior wall are areas with bone defects following previous surgery, through which infection spread to cause forehead swelling which was relieved by Antibiotics. Main complaint was severe headache.

This is a postoperative CT scan three weeks after FESS The hyperplastic lining mucosa represents the spastic lymphedema which usually occurs at this week due to the surgery and resolves spontaneously. It appears via endoscopic examination as thickened polypoid mucosa.The mucosa usually looks healthy at the fourth to sixth week.

Evacuation of right orbital intraconal abscess . The periorbita was exposed at first after resection of the lamina papyracea . The periorbita was then incised to evacuate the abscess.
