Crown Replacement after Root Canal Retreatment

Patient Complaint:
The patient presented with a fractured porcelain crown on the maxillary right central and lateral incisors, affecting both aesthetics and biting function.

Clinical Examination:
• Maxillary right central and lateral incisors: presence of fractured porcelain crown remnants.
• Surrounding gingiva appears healthy with no acute inflammation.
• Discoloration of remaining coronal tooth structure with evidence of old root canal fillings.

Radiographic Findings:
• Previous root canal fillings with voids and decreased radiodensity.
• No clear periapical pathology detected.

Treatment Plan:
1. Removal of the old fractured crown remnants.
2. Root canal retreatment of both the right central and lateral incisors using step-back preparation and lateral condensation technique with NaOCl irrigation.
3. Post and core build-up to restore coronal structure.
4. Tooth preparation followed by placement of zirconia crowns to ensure strength and optimal aesthetics.
5. Final occlusion adjustment and follow-up after two weeks to assess adaptation to the new crowns

Facial Swelling due to Chronic Periapical Infection
Chief Complaint: Severe swelling on the right side of the face, increasing in size over the last 3 days associated with mild fever and difficulty chewing
📸 Radiographic Findings
• Tooth #4: Single canal filled, underfilled and second canal missed
• Tooth #5: Two canals present, both significantly underfilled.
• Large periapical radiolucency (outlined in yellow) extending apically and laterally

🛠️ Treatment Plan:
1. Emergency Phase:
• Drainage via access cavity or surgical if fluctuant.
• Antibiotics (e.g., Amoxicillin + Metronidazole or Clindamycin if allergic).
• Pain management.

2. Definitive Phase:
• Tooth #4: Locate and treat missed canal, extend obturation to proper length.
• Tooth #5: Thorough cleaning and obturation of both canals to working length.

• Red lines indicate actual canal path and ideal fill length
• White outline: Tooth structure.
• Yellow outline: Extent of the apical lesion.
