This case shows an extreme post-pregnancy body deformity with massive skin excess (Silakis type 4B), severe re**us diastasis and advanced gluteal deflation.
In many hands, this constellation would lead to a vertical or cross-shaped scar pattern. In this case, we deliberately achieved full correction using a purely horizontal approach.
The procedure combined a 360-degree liposuction with extended abdominoplasty and targeted fat transfer into the trochanteric depressions, restoring both abdominal tension and gluteal contour.
The final intraoperative view demonstrates a fully reconstructed abdominal wall with marked tightening and anatomical realignment.
This approach requires precise planning, experience with complex body contouring and a clear understanding of when vertical scar extensions can be safely avoided.
