30-year-old male patient presenting with Norwood IV diffuse alopecia, characterized by generalized thinning affecting the frontal hairline, temporal recessions, mid-scalp, and crown area.
The surgical plan focused on restoring these regions while preserving harmony, natural density, and long-term donor area stability. The patient presented with straight, medium-caliber brown hair — characteristics that allowed for effective coverage and balanced distribution.
A total of 3,589 grafts (9,178 hairs) were transplanted:
• 329 single grafts = 329 hairs
• 1,220 double grafts = 2,440 hairs
• 1,751 triple grafts = 5,253 hairs
• 289 quadruple grafts = 1,156 hairs
Average: 2.56 hairs per graft
Diffuse alopecia cases require especially careful planning, as both the recipient and donor areas must be thoroughly evaluated before surgery. Strategic graft selection and distribution are essential to achieve natural density while preserving donor area integrity.
