ቀይ የደም ሴሎችዎ ሰላማዊ ህይወታቸውን እየኖሩ ነው… ኦክስጅንን ተሸክመው ስራቸውን እየሰሩ ነው። በድንገት የበሽታ ተከላካይ ሴሎች ጠላት ናቸው ብሎ በመሳሳት እነሱን ማጥቃት ይጀምራል ነገር ግን ጥቃቱ በማይክሮስኮፕ ውስጥ የማይታይ ሊሆን ይችላል
ለዚህ ነው ይህ ምርመራ ያስፈለገን

የኮምብስ ምርመራ ምንድነው?
የኮምብስ ምርመራ(Antiglobulin Test) ፀረ እንግዳ አካላትን ወይም ከቀይ የደም ሴሎች ጋር የተጣበቁ ፕሮቲኖችን ያገኝልናል።
በቀላል ቃላት፡- 🧪 በሽታ የመከላከል ስርዓቱ ቀይ የደም ህዋሳትን እያጠፋ እንደሆነ ይነግረናል።
ሁለት ዓይነት ምርመራዎች አሉት

1️⃣ Direct ኮምብስ ምርመራ(DAT) ቀደም ሲል በታካሚው አካል ውስጥ ከ RBCs ጋር የተጣበቁ ፀረ እንግዳ አካላትን ያገኛል።

ለመመርመር ነው

2️⃣ Indirect ኩምብ ምርመራ(IAT) ቀይ የደም ህዋሶችን ሊያጠቁ የሚችሉ በሴረም ውስጥ የሚንሳፈፉ ነፃ ፀረ እንግዳ አካላትን ያገኛል።

Your red blood cells are living their peaceful life… carrying oxygen, doing their job. Suddenly, the immune system mistakes them for enemies and starts attacking them.

That’s where the Coombs Test comes in the lab detective that reveals hidden antibodies attacking red blood cells.

What is the Coombs Test?
The Coombs test (Antiglobulin Test) detects antibodies or complement proteins attached to red blood cells.
In simple words:
🧪 It tells us if the immune system is destroying RBCs.

Two Types of Coombs Test

1️⃣ Direct Coombs Test (DAT)
Detects antibodies already attached to RBCs inside the patient’s body.
Used in:
Autoimmune hemolytic anemia

🩸 የHbA1c፣ የFBS እና የRBS ልዩነት

የትኛው የደም ስኳር ምርመራ የተሻለ ነው?
ብዙ ሰዎች “ዶክተር፣ የትኛውን የስኳር ምርመራ ላድርግ?” ብለው ይጠይቃሉ። እውነታው ግን እያንዳንዱ ምርመራ ስለ ደምዎ ስኳር መጠን የሚነግረን የተለያየ ታሪክ አለው።

1️⃣ FBS – በባዶ ሆድ የሚደረግ ምርመራ (Fasting Blood Sugar)
(ጠዋት ምግብ ሳይበሉ የሚለካ የስኳር መጠን)
ይህ ምርመራ የሚከናወነው ለ8–12 ሰዓታት ያህል ምንም አይነት ምግብ ሳይመገቡ ሲቆዩ ነው። ሰውነትዎ በዕረፍት ላይ በሚሆንበት ጊዜ የስኳር መጠንን እንዴት እንደሚቆጣጠር ያሳያል።
* 📌 መደበኛ (Normal): 70–99 mg/dL
* 📌 ለስኳር በሽታ ተጋላጭ (Prediabetes): 100–125 mg/dL
* 📌 የስኳር በሽታ (Diabetes): ≥126 mg/dL (በሁለት ተከታታይ ምርመራዎች ከተረጋገጠ)
💡 እንደ ምሳሌ፦ FBSን በአንድ የተወሰነ ቅጽበት እንደሚነሳ ፎቶ አድርገው ይቁጠሩት።
⚠️ ችግሩ ግን፦ በምርመራው ዋዜማ ተጨንቀው ከሆነ፣ እንቅልፍ ካጡ ወይም በድብቅ ጣፋጭ ሻይ ጠጥተው ካደሩ ውጤቱ ሊለዋወጥ ይችላል። ስለዚህ ጠቃሚ ቢሆንም የአንድ ወቅት ምስል ብቻ ነው።

2️⃣ RBS – በማንኛውም ሰዓት የሚደረግ ምርመራ (Random Blood Sugar)
(በማንኛውም የቀኑ ሰዓት የሚለካ የስኳር መጠን)
ይህ ምርመራ ምግብ መብላት ወይም አለመብላትዎን ግምት ውስጥ ሳያስገባ በማንኛውም ሰዓት ይከናወናል። በተለይ ለሚከተሉት ሁኔታዎች በጣም ጠቃሚ ነው፦
* ለድንገተኛ አደጋ ሕክምና ክፍሎች
* ከፍተኛ የውሃ ጥማት፣ አዘውትሮ መሽናት እና ድካም ያሉ ምልክቶች ለሚታዩባቸው ታካሚዎች
📌 ውጤቱ ≥ 200 mg/dL ሆኖ ከላይ የተጠቀሱት ምልክቶች ካሉ → በስኳር በሽታ የመያዝ ዕድልን በከፍተኛ ሁኔታ ያመለክታል።
💡 እንደ ምሳሌ፦ RBSን በማንኛውም ሰዓት ያለን የትራፊክ ፍሰት ድንገት ከመፈተሽ ጋር ይመሳሰላል።
አንዳንዴ ምግብ ስለበሉ ብቻ ሊጨምር ይችላል፤ አንዳንዴ ደግሞ በእውነትም ችግር ስላለ ከፍ ይላል። ጠቃሚ ቢሆንም ለብቻው ምርመራውን ለማረጋገጥ በቂ አይደለም።

3️⃣ HbA1c – ባለ ሶስት ወር አማካይ የስኳር መጠን (Glycated Hemoglobin)
(የ3 ወራት የደም ስኳር ትውስታ)
ይህ ምርመራ ከሌሎቹ የተለየ ነው። በደም ውስጥ ያለውን የዚያኑ ቅጽበት ስኳር ከመለካት ይልቅ፣ ባለፉት 3 ወራት ውስጥ በደማችን ቀይ ሕዋሳት (Red Blood Cells) ላይ ተጣብቆ የቆየውን የስኳር መጠን ይለካል።
ለምን 3 ወር ተመረጠ? ምክንያቱም የቀይ የደም ሕዋሳት ዕድሜ በአማካይ 120 ቀናት (4 ወራት) ስለሆነ ነው።
* 📌 መደበኛ (Normal): ከ5.7% በታች
* 📌 ለስኳር በሽታ ተጋላጭ (Prediabetes): 5.7–6.4%
* 📌 የስኳር በሽታ (Diabetes): 6.5% እና ከዚያ በላይ
💡 እንደ ምሳሌ፦ HbA1c የሰውነትዎ የባለፉት 3 ወራት "የውጤት ካርድ" እንደማለት ነው።

Which blood sugar test is actually better?
Many people ask: “Doctor, which sugar test should I do?”
The truth?
Each test tells a different story about your blood sugar.

1️⃣ FBS – Fasting Blood Sugar
(Your sugar level in the morning before eating)
This test is done after 8–12 hours without food.
It shows how your body controls sugar when it’s in a “resting state.”
📌 Normal: 70–99 mg/dL
📌 Prediabetes: 100–125 mg/dL
📌 Diabetes: ≥126 mg/dL (on two separate tests)
💡 Think of FBS like: Taking a photo of your blood sugar at one specific moment.
⚠️ But here’s the problem: If you were stressed, didn’t sleep well, or secretly drank tea with sugar at night… the result may change.
So it’s helpful but it’s just one snapshot.

2️⃣ RBS – Random Blood Sugar
(Sugar level at any time of the day)
This test can be done anytime, whether you ate or not.
It’s very useful in:
Emergency rooms
Patients with symptoms like excessive thirst, urination, weakness
📌 If RBS ≥ 200 mg/dL + symptoms → strongly suggests diabetes
💡 Think of RBS like: Checking traffic at any random time of the day.
Sometimes it’s high because you just ate. Sometimes it’s high because there’s a real problem.
It’s useful but not enough alone for diagnosis.

3️⃣ HbA1c – Glycated Hemoglobin
(Your 3-month sugar memory)
This one is different.
Instead of measuring sugar in the blood at that moment…
It measures how much sugar has been attached to your red blood cells over the last 3 months.
Why 3 months?
Because red blood cells live about 120 days.

በዚህ ግኝት ደህንነቱ የተጠበቀ የደም ልገሳ ማከናወን የተቻለ ሲሆን በዚህ ምክንያት በሚሊዮኖች የሚቆጠሩ ህይወቶችን መትረፍ ችሏል።

OGTT ምንድን ነው?
OGTT ሰውነታችን በጊዜ ሂደት ስኳርን እንዴት እንደሚያስተናግድ የሚያሳይ የላብራቶሪ ምርመራ ነው።
የምርመራው ዋነኛ ዓላማ፦
👉 ስኳር ወደ ሰውነትዎ ከገባ በኋላ... ስርአተ-ሰውነትዎ ቁጥጥሩን ይጠብቃል ወይስ ይቸገራል? የሚለውን ለማወቅ ነው።
OGTT ለምን ይደረጋል?
OGTT መደበኛ ምርመራዎች ሊዘሏቸው የሚችሉትን ነገሮች ፍንጭ ይሰጣል፦
✔ የሽንት ስኳር በሽታን (Diabetes Mellitus) ለመመርመር
✔ ቅድመ-ስኳር (Prediabetes) በሽታን አስቀድሞ ለማወቅ
✔ በእርግዝና ወቅት የሚከሰት ስኳርን (GDM) ለመለየት
✔ ያልተጠበቀ ከፍተኛ የጾም ስኳር ውጤትን በዝርዝር ለመረዳት

ሳይንሳዊ ሂደቱ 🧠
የተለካ የግሉኮስ ፈሳሽ ከጠጡ በኋላ፣ የደም ስኳር መጠን በተወሰኑ የሰዓት ልዩነቶች ይለካል።
በጤናማ ሰውነት ውስጥ፦ ኢንሱሊን ምላሽ ይሰጣል → ግሉኮስ ወደ ሴሎች ይገባል → የደም ስኳር መጠን በተፈጥሮ ይቀንሳል።
ኢንሱሊን በአግባቡ ምላሽ በማይሰጥበት ጊዜ፦ ግሉኮስ በደም ውስጥ ይቆያል → መጠኑ እንደጨመረ ይቆያል → መደበኛ ያልሆነ የምርመራ ውጤት ይታያል።
የምርመራው ሂደት (ለአዋቂዎች መደበኛ 75 ግራም OGTT)
ሂደቱ ቀላል ቢሆንም የጊዜ አጠቃቀም ወሳኝ ነው።
ከ8 እስከ 12 ሰዓታት መጾም (ውሃ ብቻ ይፈቀዳል)
የመጀመሪያውን የጾም ደም ናሙና መስጠት
በውሃ ውስጥ የሟሟ 75 ግራም ግሉኮስ መጠጣት (በ5 ደቂቃ ውስጥ)
የደም ናሙናዎች በሚከተሉት ጊዜያት ይወሰዳሉ፦
ከ1 ሰዓት በኋላ
ከ2 ሰዓት በኋላ
በምርመራው ወቅት፦
✔ በዝምታ መቀመጥ
✖ ምግብ አለመመገብ
✖ ሲጋራ አለማጨስ
✖ የአካል ብቃት እንቅስቃሴ አለማድረግ
ሰውነትዎ ትክክለኛውን ምላሽ እንዲሰጥ እረፍት ያስፈልገዋል።
የውጤት ትርጓሜ (በዓለም ጤና ድርጅት - WHO ስታንዳርድ)
ጤናማ (Normal): ከ2 ሰዓት በኋላ ያለው ስኳር < 140 mg/dL
ቅድመ-ስኳር (IGT): በ 140–199 mg/dL መካከል
የስኳር በሽታ (Diabetes): ≥ 200 mg/dL
📌 ማሳሰቢያ፦ በእርግዝና ወቅት ለሚደረግ ምርመራ (GDM) የተለየ የውጤት መለኪያ ጥቅም ላይ ይውላል።
ለጥንቃቄ የሚረዱ ነጥቦች
ታካሚው ከአጣዳፊ ሕመም ነጻ መሆን አለበት።
አንዳንድ መድኃኒቶች (ለምሳሌ ስቴሮይድ/Steroids) በውጤቱ ላይ ተጽዕኖ ሊያሳድሩ ይችላሉ።

What is OGTT?
OGTT is a lab test that shows how your body deals with sugar over time.
Simple question behind it all:
👉 After sugar enters your body… does your system stay in control, or does it struggle?

Why is OGTT done?
OGTT helps uncover what routine tests sometimes miss:
✔ To diagnose Diabetes Mellitus
✔ To catch Prediabetes early
✔ To screen for Gestational Diabetes (GDM) during pregnancy
✔ To explain unexpected high fasting glucose
Sometimes the numbers look fine at first glance.
OGTT looks deeper.

The science behind it 🧠
After drinking a measured glucose solution, blood sugar is checked at specific times.

In a healthy body:
Insulin responds → glucose moves into cells → blood sugar falls naturally.

When insulin doesn’t respond well:
Glucose stays in the blood → levels remain high → an abnormal result appears.

Procedure (75 g OGTT – standard adult)
It’s simple, but timing matters.
• Fast for 8–12 hours (water only)
• Take a fasting blood sample
• Drink 75 g glucose dissolved in water (within 5 minutes)
• Blood samples are taken at: – 1 hour
– 2 hours
During the test: ✔ Sit calmly
✖ No eating
✖ No smoking
✖ No exercise
The body needs quiet to respond honestly.

How results are interpreted (WHO simplified)
• Normal: 2-hour glucose < 140 mg/dL
• Prediabetes (IGT): 140–199 mg/dL
• Diabetes: ≥ 200 mg/dL

📌 In pregnancy, different cut-offs apply (GDM protocol).

• The patient should be free from acute illness
• Some drugs (like steroids) can affect results
• Exact timing matters minutes matter
• One small mistake can change the whole picture
Accuracy is everything.
