THE ROLE OF PHYSIOTHERAPY IN DIABETES MANAGEMENT IN AFRICA

Diabetes is a growing health concern in Africa, with the International Diabetes Federation (IDF) estimating that the number of people living with diabetes in the region will increase from 19 million in 2019 to 47 million by 2045. While medical management of diabetes is crucial, physiotherapy can play a significant role in helping individuals with diabetes manage their condition and improve their overall quality of life.

The Impact of Diabetes on Physical Function

Diabetes can have a significant impact on physical function, particularly if left unmanaged or poorly managed. Complications such as neuropathy, peripheral artery disease, and foot ulcers can lead to mobility issues, balance problems, and falls. Additionally, people with diabetes may experience muscle weakness, decreased flexibility, and reduced cardiovascular fitness.

The Role of Physiotherapy in Diabetes Management

Physiotherapy can help individuals with diabetes manage their condition and improve their physical function in several ways:

1. Exercise programs: Physiotherapists can design exercise programs that are tailored to the individual's needs and abilities, helping to improve cardiovascular fitness, strength, flexibility, and balance.

2. Foot care: Physiotherapists can provide education on proper foot care, including how to inspect feet daily, trim toenails safely, and use proper footwear.

3. Pain management: Physiotherapists can use various techniques, such as acupuncture, massage, and electrical stimulation, to help manage pain associated with diabetic neuropathy.

4. Mobility and balance training: Physiotherapists can help individuals with diabetes improve their mobility and balance, reducing the risk of falls and related injuries.

5. Education and lifestyle modification: Physiotherapists can provide education on healthy lifestyle habits, such as regular exercise, healthy eating, and stress management, to help individuals with diabetes manage their condition.

CHALLENGES AND OPPORTUNITIES IN AFRICA

While physiotherapy can play a significant role in diabetes management in Africa, there are several challenges that need to be addressed, including:

1. Limited access to physiotherapy services: In many African countries, physiotherapy services are limited, particularly in rural areas.
2. Lack of awareness: There is a need to raise awareness about the importance of physiotherapy in diabetes management among healthcare professionals and the general public.
3. Cultural and socioeconomic barriers: Cultural and socioeconomic barriers can limit access to physiotherapy services, particularly for vulnerable populations.

Despite these challenges, there are opportunities to improve access to physiotherapy services for individuals with diabetes in Africa. These include:

1. Telephysiotherapy: Telephysiotherapy can increase access to physiotherapy services, particularly in rural areas.
2. Community-based physiotherapy programs: Community-based physiotherapy programs can provide education and exercise programs for individuals with diabetes, promoting healthy lifestyle habits and improving physical function.
3. Collaboration with healthcare professionals: Physiotherapists can collaborate with healthcare professionals, including doctors and nurses, to provide comprehensive care for individuals with diabetes.

DIABETES RESEARCH UPDATE

Recent studies have shed light on diabetes in
Africa, particularly in Uganda. A notable finding is that lean individuals can develop type 2 diabetes, contrary to the common assumption that only obese people are at risk. In fact, a study conducted in seven tertiary hospitals in Uganda revealed that a third of adult patients with a recent diagnosis of diabetes had a normal body mass index (BMI).¹

This study, known as the Uganda Diabetes Phenotype (UDIP) research, also found that diabetes among lean individuals was more common in men and was predominantly associated with reduced insulin secretion by the pancreas. The researchers suggest that environmental factors, such as tropical infections and malnutrition, or genetic factors may contribute to this phenomenon.

Another significant finding is that the manifestation of type 2 diabetes in Africa differs from that in Caucasian populations in high-income countries. This underscores the need for local research to inform treatment guidelines and therapies tailored to the African population.

In terms of new treatments, researchers are exploring innovative approaches, such as immunotherapy to prevent immune cells from attacking insulin-producing beta-cells, and potential therapies for diabetic neuropathy.²

The global diabetes community came together on 14 November to support our World Diabetes Day campaign, “For a better diabetes life”, calling for well-being to be put at the heart of diabetes care. © The ADHA ASSOCIATION aims to create a better understanding amongst all people in Africa about the condition of diabetes and/or hypertension through sensitization in order to empower all people who may have diabetes and/or hypertension themselves or who are associated with people who have these pathologies to manage their condition, in order to improve the education and health services related to diabetes and/or hypertension where they are insufficient.

© We also give assistance to the neediest people of our social environment who have diabetes and/or hypertension, with special attention paid to orphan children and aged persons.
