Thanks to my colleagues, we have finally answered a recurrent question from clinicians taking care of severest neonatal RDS. When should we go for an ultrasound-guided surfactant re-treatment? It was very complex to answer as we needed many patients given the low prevalence of retreatment with optimal resoiratory care. Thus we needed a multi center design with centers having exactly the same respiratory care policy and the right timing for a second poractant-alpha dose (see our paper in Intensive Care Medicine 2011, back in another era!)… but we did it! Lung ultrasound aeration score indicates the need for retreatment with the same accuracy and same threshold of the first dose but only if performed about 12h from the first dose! 705 extremely preterm patients, in three academic NICUs in 🇪🇺(🇫🇷🇮🇹🇪🇸) published on The Lancet EBiomedicine, open access available at:

ESICM-ESPNIC evidence - based guidelines for quantitative lung ultrasound have just been published! Two years ago, the European Society of Intensive Care Medicine (ESICM) and the European Society of Paediatric and Neonatal Intensive Care (ESPNIC), agreed to create an interdisciplinary panel of experts to work on the creation of the first, evidence - based, international consensus guidelines on the use of quantitative lung ultrasound in adults, children and neonates.

Recognising the importance of the technique and its quick development, ESICM and ESPNIC have wanted to create an evidence-based framework to optimise its use and avoid wrongful interpretations. The cross-disciplinarity has been a strong added value and we believe in it! This work is set to guide the clinical practice of all intensivists for many years ahead and has been published on Intensive Care Medicine:

Fresh off press: with a multicentre collaboration between France, Spain and Italy we demonstrated that a simple lung ultrasound score (6 chest areas with no posterior scan) is not less accurate than an extended score to guide surfactant replacement in the first 24h of life. This is important as 1 save times, 2 shorten the learning process, 3 is in line with data from the adult world that will be described in the ESICM-ESPNIC consensus on quantitative lung ultrasound.

It takes only few minutes to ensure a personalized surfactant therapy!

Extended scores remains important after the first hours of life and for neonatal heterogeneous lung injuries (ie others than hyaline membrane disease).

What is the optimal CPAP level for a neonate with respiratory failure ? Unknown for sure since decades. Finally we have been able to enlighten that with quantitative lung ultrasound and comprehensive oxygenation monitoring: for babies with TTN increasing CPAP up to 8 improves lung aeration and oxygenation. However for patients with RDS, improvement was much less evident, not visible in half of patients and anyway snall in absolute values to the point to be clinically insignificant. Bottom line: if RDS is the diagnosis there is no point in increasing CPAP beyond 6. This may not improve the situation and potentially delay surfactant replacement.

Lung Ultrasound in NICU/PICU Course

Lung ultrasound is an increasingly used technique with several advantages. For the last three years, ESPNIC members have been organizing a successful two-day course on lung ultrasound in PICU and NICU in Paris. The course has been attended by medical professions from many European countries, Canada, South Africa, and China. The same course is given in English, French or Italian in different editions and consists of both theoretical, practical and interactive sessions. Didactic material is given and attendees have the possibility to do practical exercises on volunteers and neonates. The course, directed by Nadya Yousef (MD) and Daniele De Luca (MD, PhD, - ESPNIC Secretary) has also been reformatted to be offered in other hosting institutions. Additionally, instructors are available to follow trainees and give advice after the course.

Lung ultrasound in PICU/NICU Course is ESPNIC (European Society of Paediatric and Neonatal Intensive Care) endorsed.
